Diet attrition and failure of long term treatment are very frequent in obese patients. This study aimed to identify pre-treatment variables determining dropout and to customise the characteristics of those most likely to abandon the program before treatment, thus making it possible to modify the therapy to increase compliance. A total of 146 outpatients were consecutively enrolled; 73 patients followed a prescriptive diet while 73 followed a novel brief group Cognitive Behavioural Treatment (CBT) in addition to prescriptive diet. The two interventions lasted for six months. Anthropometric, demographic, psychological parameters and feeding behaviour were assessed, the last two with the Italian instrument VCAO Ansisa; than, a semi-structured interview was performed on motivation to lose weight. To identify the baseline dropout risk factors among these parameters, univariate and multivariate logistic models were used. Comparison of the results in the two different treatments showed a higher attrition rate in CBT group, despite no statistically significant difference between the two treatment arms (P = 0.127). Dropout patients did not differ significantly from those who did not dropout with regards to sex, age, Body Mass Index (BMI), history of cycling, education, work and marriage. Regardless of weight loss, the most important factor that determines the dropout appears to be a high level of stress revealed by General Health Questionnaire-28 items (GHQ-28) score within VCAO test. The identification of hindering factors during the assessment is fundamental to reduce the dropout risk. For subjects at risk, it would be useful to dedicate a stress management program before beginning a dietary restriction.

Summary: This study evaluated the effects of a 12-week online supervised training program on cardiac morphology, function, and blood pressure in children with obesity. The training program resulted in significant reductions in blood pressure parameters and detected changes in cardiac morphology. Physical activity plays a decisive role in improving blood pressure control and left ventricle systolic function, representing a strategic approach to limit cardiovascular risk.

Summary: Allergy to natural rubber latex is a prevalent issue in professional groups and the general population, with residual latex proteins being the main cause. The cross-reactivity between latex proteins and food allergens is a major concern. Purified allergens are helpful in patient management and determining specific profiles.

Summary: A study was conducted to investigate the prevalence, outcomes, and impact of COVID-19 in patients with a rare disease, such as Hereditary Hemorrhagic Telangiectasia (HHT), in the Italian population. The study found that out of 605 survey responses, 107 cases of COVID-19 were reported. Most patients had a mild course of the disease, with only eight cases requiring hospitalization and no fatal outcomes. There was no significant difference in infection risk and outcomes between HHT patients and the general population, and COVID-19 did not significantly affect HHT-related bleeding.

Neuropsychological evaluation of phenoconversion risk in REM sleep behaviour disorder: A scoping review

Giuseppe Fiamingo, Cristina Capittini, Annalisa De Silvestri, Chiara Rebuffi, Chiara Cerami, Dario Arnaldi, Michele Terzaghi

Summary: The study aimed to assess the role of cognitive evaluation in predicting phenoconversion in patients with polysomnography-confirmed idiopathic or isolated rapid eye movement sleep behavior disorder. A scoping review was conducted, and 20 selected studies were analyzed. The results showed that cognitive evaluation played an important role in predicting phenoconversion, but there were methodological differences among the studies, indicating the need for a standardized framework for acquiring and presenting neuropsychological data.

Summary: This article systematically reviewed the epidemiology of allergic rhinitis in the pediatric population over the past 10 years, revealing a significant increasing trend in the overall prevalence of diagnosed allergic rhinitis. Further investigations are needed to explore the incidence, comorbidities, diagnosis, and treatment, in order to provide a comprehensive understanding of the disease and its burden and management.

Summary: This study aimed to evaluate the outcomes of subchondroplasty (SCP) in patients with symptomatic knee bone marrow lesions (BMLs). The results showed that more than half of the patients achieved a reduction in pain after SCP, and functional scores also improved. However, a small number of patients required knee replacement due to worsening symptoms or osteoarthritis progression.

Summary: This retrospective observational study evaluated the efficacy of oral propranolol in 44 patients with infantile hemangiomas. The study found that 90.7% of cases achieved complete remission with a mean improvement in severity of 94.94%. Propranolol showed high efficacy in terms of safety profile and cosmetic results. Prematurity and precocious therapy may be associated with a better response.

Summary: This study aimed to evaluate the diagnostic delay and risk factors of symptomatic uncomplicated diverticular disease (SUDD). The study found that 25% of patients were misdiagnosed as having irritable bowel syndrome (IBS), and previous misdiagnosis was a significant risk factor for overall and physician-dependant diagnostic delay. A high educational level was associated with a longer overall diagnostic delay, while previous abdominal surgery was associated with reduced physician-dependant diagnostic delay.

Summary: Specific learning disorders (SLDs) are the most frequently diagnosed developmental disorders in childhood. This study assessed the prevalence of SLD in pediatric patients with obesity and found that SLD was more common in patients with obesity, particularly in males.
