The current status and future perspectives of organ donation in Japan: learning from the systems in other countries

The revised Organ Transplant Law came into effect in Japan in July 2010. The law allows for organ procurement from brain-dead individuals, including children, with family consent from subjects who had not previously rejected organ donation. Nevertheless, the number of cadaveric organ donations has not increased as expected. The Spanish Model is widely known as the most successful system in the field of organ donation. The system includes an earlier referral of possible donors to the transplant coordination teams, a new family-based approach and care methods, and the development of additional training courses aimed at specific groups of professionals, which are supported by their corresponding societies. South Korea, a country which neighbors Japan, has recently succeeded in increasing the rates of organ donation by introducing several systems, such as incentive programs, an organ procurement organization, a donor registry, and a system to facilitate potential donor referral. In this review, we present the current status of organ donation in Japan and also explore various factors that may help to improve the country's low donation rate based on the experiences of other developed countries.

The outcomes of thrombotic microangiopathy after liver transplantation: A nationwide survey in Japan

Summary: This study retrospectively analyzed the data of post-transplant TMA patients in Japan, and found that the outcomes of TMA were generally poor. Clinical liver transplant tolerance: Recent topics

Summary: Immunosuppression is crucial after organ transplantation, but long-term use can lead to adverse effects. Achieving immunologic tolerance is the ultimate goal, with the liver being recognized as a tolerogenic organ. Various methods, including elective weaning of immunosuppression and cell therapy, have been introduced to induce immunologic tolerance in clinical liver transplantation.

Summary: The acceptance of endoscopic surgery as a less invasive type of surgery in Japan has led to an increase in the number of endoscopic procedures performed in all surgical domains. The Japan Society for Endoscopic Surgery plays a key role in the development of endoscopic surgery by establishing a technical skills certification system for surgeons and conducting national surveys regularly.

No impact of donor sex on the recurrence of hepatocellular carcinoma after liver transplantation

Summary: This study aimed to verify whether female donors reduced the risk of hepatocellular carcinoma (HCC) recurrence after liver transplantation (LT). Evaluating 1118 recipients, the study found that donor sex did not significantly affect HCC recurrence. This suggests that donor sex should not be considered in the process of donor selection or organ allocation for liver transplantation.

Liver transplantation for intrahepatic and hilar cholangiocellular carcinoma: Most recent updates in the literature

Summary: Liver transplantation for non-hepatocellular carcinoma is still debatable, while liver transplantation for hilar cholangiocellular carcinoma should be carefully considered. For intrahepatic cholangiocellular carcinoma, transplantation should be limited to a single tumor less than 2 cm in diameter. For hilar cholangiocellular carcinoma, long-term survival can be achieved with pre-transplant chemoradiotherapy and careful selection criteria.

Long-term outcomes of living-donor liver transplantation, hepatic resection, and local therapy for hepatocellular carcinoma with three <3-cm nodules in a single institute

Summary: This study compared the outcomes of local therapy, hepatic resection, and living-donor liver transplantation for small hepatocellular carcinoma (HCC). The results showed that living-donor liver transplantation had better overall survival and cancer-specific survival rates compared to local therapy and hepatic resection, specifically for patients with HCC nodules smaller than 3 cm and Child-Pugh Grade B.

Allogeneic transplantation of epidermal cell sheets followed by endoscopic submucosal dissection to prevent severe esophageal stricture in a porcine model

Summary: This study investigated the potential of endoscopic transplantation of allogeneic epidermal cell sheets (ECSs) in preventing esophageal strictures after circumferential endoscopic submucosal dissection (ESD) using a porcine model. The results showed that transplanting allogeneic ECSs promoted mucosal healing and angiogenesis, and prevented excessive inflammation and granulation tissue formation, thus preventing esophageal strictures after ESD.

Summary: This study evaluated the surgical and oncological outcomes of laparoscopic procedures for suspected gallbladder carcinoma. The data of 129 patients with suspected GBC who underwent laparoscopic radical cholecystectomy before 2020 in Japan were analyzed. The results showed that laparoscopic radical cholecystectomy has favorable outcomes in selected patients with suspected GBC.

A hybrid procedure of living donor liver transplantation for a pediatric patient with citrin deficiency

Summary: This study investigated liver allografts from brain-dead donors that were declined and not transplanted due to various marginal factors in Japan. The results showed that pathological abnormalities of steatosis and fibrosis were the most common factors leading to transplant rejection. Although allografts with moderate steatosis were highly declined, they achieved promising outcomes after transplantation.

Summary: This study summarizes the data of gastroenterological surgery in the National Clinical Database (NCD) of Japan from 2011 to 2020. It found that the number of surgeries has been increasing in the past decade, except for a decrease in 2020 due to the COVID-19 pandemic. The average age of surgical patients is steadily increasing, with more than 20% of patients aged 80 years or older. Overall, Japanese gastroenterological surgeries have shown good safety.

The clinical impact of COVID-19 on endoscopic surgery in Japan: Analysis of data from the National Clinical Database

Summary: This study evaluated the impact of COVID-19 on elective endoscopic surgeries in Japan. The number of surgeries, including laparoscopic cholecystectomy, laparoscopic distal gastrectomy, and laparoscopic low anterior resection, decreased in 2020 compared to previous years. Although the use of robot-assisted surgeries increased, the growth rate was lower than in 2019. The infection rate in different prefectures did not significantly affect the surgery numbers. The surgeries decreased from May to June but gradually recovered.

Outcomes of living donor liver transplant recipients receiving grafts with the graft-to-recipient weight ratio less than 0.6%: A matched pair analysis

Summary: This study retrospectively analyzed 280 cases of adult living donor liver transplantation (LDLT) and found that a graft-to-recipient weight ratio (GRWR) of less than 0.6% was associated with an increased risk of small-for-size syndrome (SFSS). Although overall graft survival rates were similar, appropriate donor and recipient selection is crucial for successful LDLT with very small grafts.
