Prone-Positioning for Severe Acute Respiratory Distress Syndrome Requiring Extracorporeal Membrane Oxygenation

Summary: This study analyzed the characteristics and outcomes of patients with severe acute respiratory distress syndrome who received prone-positioned extracorporeal membrane oxygenation treatment. The results showed that prone-positioning during extracorporeal membrane oxygenation was safe and effective, and was associated with a higher probability of surviving and being weaned-off ECMO at 90 days. Additionally, quantitative lung CT-scan performed before prone-positioning can help predict the improvement in static compliance after that procedure.

The 90-day mortality rate of the Chilean cohort of ECMO-supported patients with COVID-19 was found to be 38.8%, comparable to previous reports.

Extracorporeal membrane oxygenation for severe acute respiratory distress syndrome in children with leukemia/lymphoma: A retrospective case series

Summary: This study retrospectively analyzed seven cases of pediatric leukemia/lymphoma patients who developed ARDS and underwent ECMO treatment. The survival rate at hospital discharge was 42.9%, with a 6-month survival rate of 28.6%. The main ECMO-associated complication was bleeding, followed by nosocomial infection. All patients required vasopressor support and six underwent continuous renal replacement therapy (CRRT).

Summary: This study emulated a trial comparing the effect of ECMO and IMV on COVID-19 patients, and found that ECMO had higher survival probability on Day 7, but the difference decreased during follow-up. However, ECMO was consistently associated with better outcomes when performed in high-volume centers and regions with ECMO capacities specifically organized to handle high demand.

Summary: This study aimed to describe the institutional experience with extremely prolonged venovenous extracorporeal membrane oxygenation (ECMO) support for patients with acute respiratory failure. The study found that most patients in this cohort had adult respiratory distress syndrome (ARDS) secondary to COVID-19. Prolonged ECMO support was successful in weaning patients off and enabling their recovery, with a high rate of survival.

Summary: Intracranial hemorrhage is a serious complication in patients receiving venovenous extracorporeal membrane oxygenation during treatment of the acute respiratory distress syndrome. The study found that early occurrence of intracranial hemorrhage is associated with higher 60-day mortality, and adjusting modifiable risk factors may lower the prevalence of this complication.

Summary: This study assesses the association of the timing of prone positioning (PP) during venovenous extracorporeal membrane oxygenation (V-V ECMO) with the probability of being discharged alive from ICU and improvement of respiratory system compliance. The results show that delayed PP during V-V ECMO is associated with lower ICU discharge probability and less improvement in respiratory system compliance. Early PP is beneficial for ARDS patients on ECMO.

Summary: This study analyzed the characteristics and outcomes of veno-venous (V-V) extracorporeal membrane oxygenation (ECMO) for acute respiratory distress syndrome (ARDS) due to COVID-19 versus non-COVID causes. The results showed that patients with COVID-19 ARDS requiring V-V ECMO support had higher in-hospital mortality, longer length of stay, and higher direct hospitalization cost compared to non-COVID etiologies.

Cytokine Adsorption in Severe Acute Respiratory Failure Requiring Veno-Venous Extracorporeal Membrane Oxygenation

Summary: This study reports on nine severe ARDS patients treated with V-V ECMO and CytoSorb adsorber, compared with a control group of nine similarly treated patients without cytokine adsorption. Despite prediction models suggesting higher mortality in the cytokine adsorption group, patients undergoing V-V ECMO and cytokine removal had reduced mortality compared to those receiving V-V ECMO alone. The cytokine adsorption group showed significant improvements in fluid resuscitation, vasopressor support, and lactate levels within 72 hours, indicating potential benefits for patients with severe ARDS requiring V-V ECMO support.

Summary: High-throughput proteomics was used to explore the roles of biomarkers in diabetes pathophysiology. The study identified novel protein contributors to glucose homeostasis and diabetes risk through proteomic association studies. 51 proteins were characterized to be associated with longitudinal diabetes risk, including beta-glucuronidase, plexin-B2, alcohol dehydrogenase-1C, fructose-bisphosphate aldolase-B, sorbitol dehydrogenase, leucine-rich repeat containing protein-15, and myocilin.

Summary: This study explores the relationship between healthy dietary patterns and physical fitness in adults, as well as the biological connections between diet and fitness through circulating metabolites. The results indicate that a healthy diet is associated with greater physical fitness in middle-aged adults, and this relationship may be explained by favorable metabolic health.

Summary: This study explores the genetic and biological pathways associated with submaximal exercise capacity, specifically focusing on intrinsic capacity and its response to endurance training. By using a combination of genomics, gene expression, proteomics, and metabolomics experiments, the researchers identified panels of genes associated with intrinsic capacity and trainability. The findings suggest that variations in submaximal exercise capacity are primarily driven by skeletal muscle morphology and metabolism, as well as red blood cell oxygen-carrying capacity.
