Summary: This study retrospectively reviewed the tumor characteristics and surgical outcomes of a consecutive series of endoscopic endonasal approach pituitary adenomas resection mainly performed by a single author between March 2018 and June 2019. The results demonstrated that EEA is an effective and safe approach for resecting pituitary adenomas with satisfactory gross total resection rate, near-total resection rate, and endocrinological remission rate, while maintaining minimal nasal injury.

Summary: This study evaluated the outcomes of the endoscopic transsphenoidal approach as the primary treatment for somatotroph adenomas and investigated patients with suboptimal surgical results. Factors such as tumor size, preoperative GH level, and residual tumors were found to be associated with surgical failure. Octreotide showed good outcomes for treating DGSA patients, while SRS/EXRT generated good results for patients receiving secondary treatments when remission could not be reached after 6 months of TSA operation.

Summary: Recovery from AGHD after initial TSS is more likely in younger female patients with higher peak GH levels assessed by preoperative PPT. Patients aged <= 62.2 years and with a peak GH level of >= 0.74 μg/L based on preoperative PPT are likely to recover from AGHD.

Summary: This study aims to investigate the incidence and predictors of delayed postoperative hyponatremia (DPH) in acromegaly patients undergoing transsphenoidal surgery. The findings suggest that lower preoperative weight and a postoperative transient gain in body weight are associated with an increased risk of DPH.

Summary: National registries are valuable tools to investigate rare conditions like acromegaly, providing insights into epidemiology, clinical presentation, diagnosis, treatment outcomes, and mortality. While single-center studies are more homogeneous, they are often limited by small sample sizes and lack statistical power.

Summary: This study investigated the prevalence and patient characteristics of acromegaly over time, as well as the impact of treatment changes on disease control. The study found that the prevalence of acromegaly has increased significantly, with a shift towards a milder clinical phenotype. Modern treatment approaches enable individualized treatment and disease control in the majority of patients with acromegaly.

Summary: This study aimed to evaluate the prevalence and determinants of morphometric vertebral fractures (VFs) in patients with recently diagnosed acromegaly. The results showed a higher prevalence of VFs in acromegaly patients compared to controls, and it was found to be associated with GH levels. Therefore, VF assessment should be included in the diagnosis of acromegaly.

Summary: Histological diagnosis of sellar masses can be challenging, especially in rare cases without definitive biomarkers. DNA methylation profiling has emerged as a useful tool in assisting diagnosis, particularly in cases of atypical presentation or diagnostic uncertainty. This case illustrates the clinical utility of machine-learning-based DNA methylation classifiers in improving diagnostic accuracy and guiding therapeutic and prognostic decisions for CNS tumors.

Summary: Intercavernous sinuses (ICSs) are important physiological communications in the brain, running between the layers of the dura mater. The inferior ICS (iICS) has been less studied, but this research found it in all cadaveric specimens, with various shapes and dimensions. Precise radiological assessment is crucial for safe surgical approaches involving the iICS.

Summary: Transsphenoidal surgery is the standard treatment for hormone-secreting pituitary adenomas. This study proposes a novel way to classify and report surgical outcomes by integrating efficacy and safety at the patient level. The results show that surgical interventions can be presented in four categories, incorporating both efficacy and safety.

Summary: This study retrospectively analyzed the outcomes of the transsphenoidal approach for nonfunctioning pituitary adenomas treated at a single institute. The study found that the transsphenoidal approach is an effective and safe treatment, improving visual impairment and restoring endocrine function. Diabetes insipidus was the most common complication, and complete resection during surgery helped to lower the risk of recurrence.

Summary: The endoscopic endonasal approach has proven to be an effective technique for treating hypothalamic-pituitary metastases, allowing for tissue sampling and neurovascular decompression, both essential for integrated adjuvant therapy protocols. The study presents the experience of a single institution with 13 cases of hypothalamic-pituitary metastasis, focusing on preoperative evaluation, surgical technique evolution, and postoperative management. Results show promising effects on progression-free and overall survival rates for this relatively rare location.

Summary: Giant pituitary adenomas present a surgical challenge due to their growth patterns and locations. There is no consensus on the optimal surgical strategy, but transcranial or transsphenoidal approaches are commonly used. Endoscopic endonasal approach is preferred, but transcranial approach is more suitable when the tumor has significant intracranial extension. Accurate preoperative evaluation and individualized treatment plans are crucial for long-term effectiveness.

Summary: This study discussed the pathological features, comorbidities, and surgical complications of intracranial meningiomas in patients aged >80 years. The results showed significant differences in meningioma types, Ki-67 MIB1 expression, progesterone receptor expression, and rates of postoperative neurological deficits and acute bronchopneumonia in patients aged >80 years. Therefore, careful patient selection is necessary, and surgery should not be delayed due to advancing age.

Summary: This study compared the anatomy of the cavernous sinus from different surgical approaches and identified corresponding landmarks along these routes. The results showed that different surgical corridors have advantages in exposing different regions of the cavernous sinus, and the selection of approach should be guided by the relationship between the tumor and cranial nerves. The transorbital endoscopic approach can be considered a safe and minimally invasive surgical corridor for selected lesions of the cavernous sinus, in combination with transcranial and endoscopic endonasal approaches.

Summary: Large populations in middle- and low-income countries in Africa, Asia, and Central and South Americas, as well as large social groups in high-income countries, face limited access to neurosurgical services. This study evaluates if special social groups in Greece face similar problems and identifies socio-economic factors, language barriers, cultural and religion differences, geographical barriers, the COVID-19 pandemic aftermath, and malfunctioning of the Greek health system as underlying causes. The study proposes reforms such as redrawing the health map, reorganizing the national health system, and implementing telemedicine to alleviate the neurosurgical crisis both at a local and global level.

This study examines the impact of deep brain stimulation on the sagittal balance of Parkinson's disease patients with abnormal forward flexion of the spine. The results show significant improvement in dorsal kyphosis angle after 12 months of deep brain stimulation treatment. However, further larger studies are needed to confirm these findings.
