Summary: This article reports a rare case of bilateral fenestration of the vertebral artery (FVA) in the craniocervical junction (CCJ) region. Three-dimensional computed tomography angiography (3D-CTA) was found to be more sensitive than conventional magnetic resonance angiography in detecting the FVAs and depicting their relationships with surrounding bony structures. 3D-CTA is essential for endovascular and open surgical interventions in the CCJ region.

Summary: This study aimed to investigate the influence of arterial remodeling patterns on plaque characteristics and postoperative outcomes in patients with severe basilar artery stenosis. The results showed that remodeling patterns did not significantly affect the outcome of ischemic events and new ischemic cerebral lesions. However, the remodeling index was strongly associated with plaque burden, providing new insights for the evaluation of severe basilar artery stenosis.
