Impact of White Matter Hyperintensity Burden on Outcome in Large-Vessel Occlusion Stroke

Summary: This study aimed to assess whether the burden of white matter hyperintensity (WMH) is associated with an increased risk of symptomatic intracranial hemorrhage (sICH) or poor functional outcome in patients with acute ischemic stroke treated with endovascular thrombectomy (EVT). The results showed that a higher burden of WMH was associated with an increased risk of poor functional outcome at 3 months after EVT.

Hemorrhagic risk after intravenous thrombolysis for ischemic stroke in patients with cerebral microbleeds and white matter disease

Summary: The study aimed to evaluate the association between cerebral microbleeds (CMBs) and white matter disease (WMD) with intracerebral hemorrhage (ICH) after IV thrombolysis (IVT) with rt-PA. Results suggest that excluding eligible candidates for IV rt-PA treatment solely based on the presence of CMBs is not justified. Clinical decisions should be made on a case-by-case basis and more data is needed to assess the benefit-risk profile of rt-PA in patients with numerous microbleeds.

Summary: White matter hyperintensity volume is modestly positively associated with elevated amyloid in older adults without dementia, with a stronger association observed among Black participants. However, there was no association between white matter microstructure and elevated amyloid. These findings need to be further validated in larger, multiethnic cohorts.

Effect of intravenous thrombolysis in acute ischemic stroke patients with cerebral microbleeds and analysis of risk factors for hemorrhagic transformation

Summary: The safety and efficacy of intravenous thrombolysis with rt-PA in patients with acute ischemic stroke and cerebral microbleeds were investigated, and the risk factors for hemorrhagic transformation were analyzed. The results showed that intravenous thrombolysis with rt-PA significantly improved short-term neurological function recovery and long-term prognosis, with no statistically significant differences in the incidence and mortality rates of ICH. Furthermore, the time from stroke onset to thrombolysis, baseline NIHSS score, and history of atrial fibrillation were identified as independent risk factors for hemorrhagic transformation in patients treated with intravenous thrombolysis.

The study suggests that the activation of the PIK3CA pathway may be a possible universal regulator of vascular morphogenesis, but conflicting data exist among different groups and studies.

Diabetes and Ischemic Stroke: An Old and New Relationship an Overview of the Close Interaction between These Diseases

Summary: Diabetes and atherosclerosis are two complementary diseases that interact through multiple molecular mechanisms, increasing the risk of cardio- and cerebrovascular events. Chronic hyperglycemia and advanced glycosylation end-products contribute to accelerated atherosclerosis by damaging the endothelium and impairing cellular function. Diabetic patients are more susceptible to cerebrovascular complications, especially in younger individuals and those with hypertension and other vascular disorders.

Patterns and Outcomes of Intensive Care on Acute Ischemic Stroke Patients in the US

Summary: This study aimed to summarize the proportion of elderly acute ischemic stroke (AIS) patients admitted to an intensive care unit (ICU) in the United States and assess the national availability of ICU-level care for AIS. The study found that 19.9% of AIS admissions involved ICU-level care, with younger patients more likely to receive reperfusion therapy but also having more comorbid conditions and neurologic complications. Additionally, it was discovered that not all hospitals provide ICU-level care for AIS, with teaching hospitals, those with larger AIS volumes, and those located in larger metropolitan areas more likely to have an available ICU.

The Interplay between Cardiovascular Risk, Cardiovascular Events, and Disease Activity in Primary Sjogren's Syndrome: Is Uric Acid the Missing Link?

Summary: This study found a strong association between disease activity, interstitial lung disease (ILD), and cardiovascular events in primary Sjögren's syndrome (pSS) patients. The association between ILD and cardiovascular events was dependent on higher serum uric acid levels and independent of other traditional cardiovascular risk factors. Assessing serum uric acid levels in pSS patients and considering the application of correction factors in cardiovascular risk assessment tools can achieve better stratification of cardiovascular risk.

Cerebral microbleeds in dementia with Lewy bodies

Summary: Cerebral microbleeds (CMB) are present in 30% of individuals with dementia with Lewy bodies (DLB), with a lobar predominance. DLB patients with CMB are more likely to receive antithrombotic therapy and have a history of hypertension compared to those without CMB. However, no significant differences were found in DLB core clinical features, cognition, and functional status between the two groups. The presence of CMB did not show an association with cortical amyloid-beta deposition on PET imaging in DLB patients. Further research into the pathophysiology and clinical implications of CMB in DLB is needed.
