Cathepsins are increased in the brain of elderly animals. We investigate the presence of cathepsin B and S in human cerebrospinal fluid (CSF) plasma and the associations with cystatin C, age and sex. We measured cathepsin B and S concentrations in CSFs from 118 persons, undergoing elective surgical procedures, with ELISA. Both cathepsin B and cathepsin S were positively correlated with age. No correlation was observed between cathepsin B or S and length, height or body mass index. Both cathepsin B and S were positively correlated to the cystatin C concentration in CSF. Calculated reference intervals were 4,893-17,636 pg/mL for cathepsin B and 2,681-11,459 pg/mL for cathepsin S. Elderly individuals had significantly higher levels of both cathepsin B (r (s) = 0.38, p = 0.00002) and cathepsin S (r (s) = 0.35, p = 0.0001) in CSF.

Exploring the Relationship between Frailty, Functional Status, Polypharmacy, and Quality of Life in Elderly and Middle-Aged Patients with Cardiovascular Diseases: A One-Year Follow-Up Study

Summary: This study investigated the association between frailty, disability in activities of daily living (ADL), polypharmacy, and quality of life (QoL) in middle-aged patients with cardiovascular disease (CVD). The study found that frail patients with CVD showed functional dependency and poor QoL compared to non-frail patients. Mobility was identified as a predictor of frailty in both elderly and middle-aged patients. ADL assessment and self-reported QoL may help identify an aggravation or an advanced frailty condition in hospitalized elderly and middle-aged CVD patients.

Nutritional Risk Assessment Scores Effectively Predict Mortality in Critically Ill Patients with Severe COVID-19

Summary: This study evaluated the effectiveness of four nutritional risk assessment tools and explored the predictive value of CT-derived fat tissue and muscle mass measurements in predicting in-hospital mortality of critically ill patients with COVID-19-associated ARDS. The results showed that the nutritional risk assessment tools can accurately predict mortality, while fat tissue and muscle mass were not associated with increased mortality risk.

Summary: Inflammation in COVID-19 leads to intracellular iron overload and non-programmed cell death, increasing mortality and morbidity. This study aims to evaluate the predictive values of serologic iron and inflammatory parameters in critically ill COVID-19 patients. The results suggest that iron metabolism parameters and models consisting of multiple variables may predict outcomes. Larger studies are needed to validate these results, but this paper highlights a new research direction.

Effects of Tocilizumab on Inflammation and Iron Metabolism in Critically Ill Patients with COVID-19

Summary: COVID-19 induces persistent inflammation through cytokines and disrupts iron homeostasis. Blocking the interleukin 6-hepcidin pathway with Tocilizumab does not affect survival or the occurrence of anemia or sepsis in critically ill patients. Elevated hepcidin levels and anemia after treatment suggest the involvement of alternative pathways. Further research is needed to understand these pathways and develop new therapies for persistent inflammation.

Trends in Molecular Diagnosis of Nosocomial Pneumonia Classic PCR vs. Point-of-Care PCR: A Narrative Review

Summary: Nosocomial pneumonia, particularly ventilator-associated pneumonia, is a common hospital-acquired infection that poses challenges due to multidrug-resistant pathogens. Prompt and targeted antimicrobial therapy is crucial, and rapid diagnostic techniques like polymerase chain reaction (PCR) have advantages over traditional culture-based methods. The latest development in PCR, point of care testing-PCR (POC-PCR), allows for bedside testing in intensive care units. This review aims to highlight the benefits and limitations of PCR-based techniques in managing nosocomial pneumonia.

Summary: This study found that the neutrophil-to-lymphocyte ratio (NLR), derived-NRL (d-NLR), platelet-to-lymphocyte ratio (PLR), and lymphocyte-to-C-reactive protein ratio (LCR) can predict the risk of septic shock and death in COVID-19 patients in the intensive care unit (ICU). The values of NLR, dNLR, and PLR during hospitalization can effectively predict the likelihood of developing septic shock and death.
