Comparison of acid-lowering drugs for endoscopy negative reflux disease: Systematic review and network Meta-Analysis

Summary: This study conducted a network meta-analysis to evaluate the efficacy and safety of different medical therapies for endoscopy-negative reflux disease. The results showed that proton pump inhibitors (PPIs) are superior to other drugs in treating this condition. Future research should focus on better patient classification and investigating the efficacy of other drugs.

Summary: Eosinophilic esophagitis (EoE) is a growing cause of chronic esophageal morbidity. It is characterized by eosinophilic infiltrates of the esophagus and organ dysfunction. Esophageal motor abnormalities may contribute to symptoms and should be considered in treatment and evaluation of patients with EoE.

Summary: This study found that the gluten-free diet is associated with an increased risk of weight gain and the development of metabolic disorders. Evaluating the nutritional status using specific parameters, the study found that patients with celiac disease had a poorer nutritional status at diagnosis compared to healthy controls, but their nutritional status significantly improved after following a gluten-free diet.

Encouraging appropriate use of proton pump inhibitors: existing initiatives and proposals for the future

Summary: Proton pump inhibitors (PPIs) are widely used in the treatment of acid-related disorders, but their increasing use cannot be fully explained by the prevalence of these diseases alone. Inappropriate prescribing and overuse of PPIs has led to increased costs and potential harm. This review highlights current indications, potential adverse events, misuse of PPIs, and proposes strategies for improving their use. PPI deprescribing is recommended as the preferred approach, but more research is needed to determine optimal deprescribing methods in different clinical scenarios.

Summary: This systematic review and meta-analysis evaluated the efficacy and safety of low bioavailability steroids in treating patients with mild-to-moderate ulcerative colitis. The results showed that low bioavailability steroids were more effective than placebo in achieving remission and had similar efficacy to 5-aminosalicylic acid (5-ASAs). More randomized controlled trials are needed to confirm these findings.

Summary: There is a lack of data on the effectiveness and safety of Janus kinase inhibitors and sphingosine-1-phosphate receptor modulators in antibiotic refractory chronic pouchitis (CARP). This study retrospectively collected data on JAK inhibitor or S1P receptor modulator treatments for CARP and found that small molecules may be a suitable option for CARP refractory to multiple biologics, but further investigation is warranted.
