Deep brain stimulation for dystonia - Consensus recommendations of the German Deep Brain Stimulation Association

Medical treatment of dystonia, particularly generalised forms of the disorder, is often not satisfactory or causes intolerable side effects. In focal dystonia, a reasonable treatment option with botulinum toxin exists but some patients either do not respond well or develop neutralising antibodies with secondary therapy failure. Deep brain stimulation (DBS) of the globus pallidus internus has been shown to be effective in both generalised and focal dystonia. This paper gives recommendations regarding the use of DBS in different forms of dystonia based on the currently available scientific data as well as the longstanding personal experience of the authors. The inclusion criteria for DBS candidates as well as the peri- and postoperative patient management are addressed. These recommendations were developed in a consensus procedure in the German Deep Brain Stimulation Association.

Clinical benefit of MAO-B and COMT inhibition in Parkinson's disease: practical considerations

Summary: Inhibitors of monoamine oxidase B (MAO-B) and catechol-O-methyltransferase (COMT) are important strategies in Parkinson's disease treatment. Several drugs have been available on the market for more than a decade, while others have been approved more recently. Various post-authorization studies have provided comprehensive data on their use and characteristics in real-life clinical practice. This article summarizes the current knowledge on these medications and discusses their clinical value in Parkinson's disease, as well as practical considerations and ongoing studies.

Grey matter correlates of dystonic soft signs in essential tremor

Summary: This study aims to explore alterations of brain grey matter in patients with essential tremor (ET) combined with dystonic soft signs (ET + ds). The results showed a significant increase of grey matter in the right lentiform nucleus in ET + ds and tremor associated with manifest dystonia (TAWD) compared to healthy controls and ET patients without dystonic soft signs (ET-ds). Additionally, an increase of cortical grey matter in the middle frontal gyrus was observed in ET + ds. The hypertrophy of the lentiform nucleus in ET + ds was correlated with disease severity and duration.
