The Malnutrition-Inflammation-Depression-Arteriosclerosis Complex Is Associated with an Increased Risk of Cardiovascular Disease and All-Cause Death in Chronic Hemodialysis Patients

Background: In chronic hemodialysis patients, malnutrition, inflammation, depression and arteriosclerosis are pathogenetically associated suggesting the presence of malnutrition-inflammation-depression-arteriosclerosis (MIDA) complex acting as a risk factor for cardiovascular disease (CVD). Methods: Nutritional status was assessed by serum albumin, subjective global assessment and normalized protein catabolic rate (nPCR). Inflammation was assessed by serum high-sensitivity C-reactive protein (hsCRP). Depression was assessed with the Beck Depression Inventory and DSM-IV criteria. The severity of arteriosclerosis was measured by pulse wave velocity (PWV). Results: Among 81 hemodialysis patients, 44 (54.3%) had malnutrition (albumin <4.0 mg/dl with subjective global assessment score <6 and/or nPCR <1.0) and 39 (48.1%) had inflammation (hsCRP > 1 mg/l). The prevalence of depression was 50.6% (n = 41). Fifty-nine (73.8%) had arteriosclerosis (measured PWV > expected PWV based on age/blood pressure/gender adjustment). The severity of the all four individual MIDA components correlated well with each other. The average number of the MIDA complication (MIDA score) was 2.27 +/- 1.33. During the 5-year follow-up, 40 cases of CVD and 26 cases of all-cause death occurred. In Cox analysis adjusted for previous CVD, age, diabetes, blood pressure, pulse pressure, intradialytic hypotension, B-type natriuretic peptide, hemoglobin and hemodialysis incompliance, the MIDA score was an independent predictor of CVD and all-cause death: hazard ratio (95% confidence interval); 1.89 (1.13-3.17) and 3.48 (1.32-9.21) for an increase of 1 MIDA score. Conclusions: This study suggests the presence of MIDA complex, which is composed of malnutrition, inflammation, depression and arteriosclerosis. The MIDA complex syndrome was an independent risk factor for CVD and all-cause death in chronic hemodialysis patients. Copyright (c) 2013 S. Karger AG, Basel

Internet Addiction and Its Associations with Clinical and Psychosocial Factors in Medical Students

Summary: This study identified the prevalence of internet addiction (IA) in medical students and its associations with clinical and psychosocial factors. The results showed that self-esteem was associated with a lower risk of IA, while depression and social anxiety were associated with a higher risk of IA. Strengthening self-esteem and reducing depression and social anxiety may contribute to the prevention and management of IA in medical students.

Reliability, Validity, and Unidimensionality of the Korean Version of the Pornography Craving Questionnaire Based on the Classical Test Theory and Item Response Theory

Summary: This study evaluated the reliability and validity of the Korean version of the Pornography Craving Questionnaire (K-PCQ) using classical test theory and item response theory. The results showed that all items of the K-PCQ measured a single construct, had excellent internal reliability, and were unidimensional. The K-PCQ is considered a useful screening tool for pornography craving in clinical practice and research.

Frontal EEG response to alcohol craving elicited by individually tailored video cues

Summary: This study examined the quantitative EEG activities in alcoholics and social drinkers exposed to video cues and found that alcoholics had significantly increased beta activity in the right DLPFC region. Beta activity at the F4 electrode was correlated with alcohol craving, anxiety, depression, and changes in subjective craving scores in both alcoholics and social drinkers. These findings suggest the functional importance of hyperarousal and negative emotions in craving induced by video cues.

Summary: This study comprehensively examined compulsive sexual behavior disorder (CSBD) in 42 countries, different genders, and sexual orientations, and validated the CSBD-19 and CSBD-7 scales as screening tools. Results showed that 4.8% of participants were at high risk of CSBD, with differences found based on country and gender. The study contributes to a better understanding of CSBD in underrepresented populations and provides freely accessible screening tools in 26 languages.

Social Stigma and Discrimination Toward People With Drug Addiction: A National Survey in Korea

Summary: This study aims to understand the attitudes, stigma, and discrimination of the general adult population toward drug addiction. A nationwide survey was conducted with 1,020 Korean adults, and the results showed that the majority of participants had negative perceptions of drug addiction and agreed to unfair treatment of drug addicts. Factors such as gender, age, personal experience of opioid misuse, smoking status, and income were found to be associated with stigma and discriminatory attitudes. The findings highlight the importance of education on drug addiction prevention and reducing stigma and discrimination toward drug addicts.

Relationship of Neural Correlates of Gait Characteristics and Cognitive Dysfunction in Patients with Mild Cognitive Impairment

Summary: This study found an association between gait disturbances and cognitive function in patients with mild cognitive impairment. Gait characteristics such as velocity, step count, step length, and duration of swing and stance phase were correlated with cognitive impairment. Additionally, changes in gray matter volume and curvature in specific brain areas were also associated with gait characteristics related to cognitive dysfunction. Therefore, evaluating gait and cognitive functions is important in patients with mild cognitive impairment.

Effect of a Motivational Interviewing- Based Brief Intervention on Alcohol Use Behavior in Korean Internal Medicine Settings

Summary: A motivational interviewing (MI)-based brief intervention was conducted with high-risk drinking outpatients in internal medicine settings in Korea. Participants were assigned to a MI group or a control group, with both groups showing decreased AUDIT-C scores after four weeks, although not significantly different. However, the intervention group had a greater decrease in AUDIT-C scores over time compared to the control group, suggesting that short comments from doctors may be important in brief interventions for high-risk drinking management in Korean clinical settings.
