Summary: This study used cross-sectional analysis of National Health and Nutrition Examination Survey (NHANES) data from 2009 to 2018 to investigate the relationship between opioid and cannabis use. The results showed that among those who recently used prescription opioids, only 18.4% also used cannabis, which was not significantly different from non-users. Further analysis revealed that opioid users were less likely to use cannabis, and there was no difference in cannabis use between chronic and short-term opioid users. These findings suggest that the use of opioids may not promote the use of cannabis independently.

Summary: A study conducted in Taiwan found a positive association between hormone replacement therapy (HRT) and carpal tunnel syndrome (CTS) in women aged 45 years and older. Summary: The study revealed that primary isolated hypertriglyceridemia (PIH) is associated with increased cardiovascular risk, particularly in terms of incident coronary heart disease events. Although the association with coronary heart disease remained significant even after adjusting for conventional risk factors, there was no significant association with cerebrovascular disease or peripheral artery disease. Awareness and control of PIH were found to be relatively low despite its prevalence.

Summary: Background: Acute kidney injury (AKI) is known to increase morbidity and mortality after general surgery, but its association with pancreatoduodenectomy outcomes is unclear. This study aimed to investigate the relationship between AKI and postoperative complications and death after pancreatoduodenectomy. Methods: All patients aged 18 years or older who underwent pancreatoduodenectomy at a university hospital in Stockholm, Sweden, between 2008 and 2019 were included. AKI was graded using standardized criteria such as estimated glomerular filtration rate (eGFR) and urine volume measurements. Results: Of the 970 patients included, 137 (14.1%) developed postoperative AKI. Risk factors for AKI included lower preoperative eGFR, cardiovascular disease, and treatment with renin-angiotensin system inhibitors or diuretics. Patients with AKI had a higher risk of severe postoperative complications and death within 30 days or 90 days. Furthermore, patients with benign histology and AKI had increased 1-year mortality. Conclusions: Postoperative AKI after pancreatoduodenectomy is associated with significant complications and an increased risk of mortality. Monitoring serum creatinine levels and urine output in the immediate perioperative period may improve outcomes.

Summary: This study explores the longitudinal trajectories of clinical indicators, patient-reported outcomes, and hospitalizations in older patients with advanced chronic kidney disease (CKD) before death. The results show that eGFR, Subjective Global Assessment score, and blood pressure decline, with accelerations observed 6 months before death. Serum hemoglobin, hematocrit, cholesterol, calcium, albumin, and sodium values decline slowly during follow-up, but with accelerations observed between 6 and 12 months before death. Physical and mental quality of life decline linearly, while the number of reported symptoms remains stable up to 2 years before death, accelerating at 1 year before death. The rate of hospitalization remains stable at around one hospitalization per person year, but increases exponentially 6 months before death.

Summary: Chronic kidney disease-mineral and bone disorder (CKD-MBD) is common in older non-dialysis patients with advanced CKD. PTH and phosphate are independently associated with all-cause mortality, while only PTH is associated with cardiovascular mortality. Calcium does not have an independent association with mortality but modifies the effect of phosphate.

Summary: The composition of gut microbiota can be influenced by host genetic and dietary characteristics, as well as disease state and environmental stressors. Uremic dysbiosis, characterized by a loss of biodiversity and overgrowth of microorganisms, can lead to the elevation of metabolic solutes such as TMAO, which may have pathogenic effects on the host. TMAO levels are elevated in patients with chronic kidney disease due to decreased clearance and increased production from uremic dysbiosis. Dietary precursors of TMAO can be found in animal-derived foods and certain types of seafood. Although TMAO has been linked to vascular damage and adverse cardiovascular outcomes, the evidence in CKD patients is inconsistent. This review explores the potential dietary sources of TMAO and its effects on the intestinal microbiome, as well as the role of a healthy diet in preventing dysbiosis and reducing toxin levels in CKD patients.
