From registry data collection to international comparisons: examples of haemodialysis duration and frequency

Background. The purpose of this study was to investigate haemodialysis (HD) dose practice patterns in different European countries in the light of the European Best Practice Guidelines (EBPG) and to study the associations of patient characteristics and country with weekly dialysis duration. Methods. Renal registries in Europe were asked to contribute to the study with individual patient data on weekly HD duration, number of HD sessions a week and last measured Kt/V. Additional items were age, sex, date of first renal replacement therapy (RRT), dry weight, height, HD modality, HD technique, diabetes status and vascular access type. Multivariate logistic regression was used to study the probability of receiving HD for < 12 h per week. Results. Seven registries contributed data on 26 136 patients on HD on 31 December 2005. Eighty-three percent of the patients received HD for at least 12 h per week as recommended by the EBPG (range 49.0-97.3% across countries). Multivariate analysis showed significant differences across countries concerning the risk of receiving < 12 h. Other risk factors included age (older), sex (female), BMI (low) and duration of RRT (shorter). Diabetes was associated with longer total HD duration. Conclusion. This study shows a great international variability in weekly HD duration and some discrepancies between current practices and the EBPG. It also points out the difficulty of obtaining and comparing Kt/V values under current registry practices.

Summary: This study compared patient and graft survival of kidney transplant recipients who received a kidney from a living-related donor (LRD) or living-unrelated donor (LUD). The results showed that patient and graft survival were similar between LRD and LUD transplants, but death-censored graft failure was higher in LUD transplants.

Clinical and dialysis-related characteristics of extremely long-term hemodialysis survivors: Three case reports

Summary: These long-term hemodialysis survivors initiated dialysis at a young age, were free of diabetes, maintained a stable and relatively low body weight with good nutritional status. Despite complications, they lived independently and fully with optimal management of chronic kidney disease and preventive measures for cardiovascular disease.

Summary: This study describes the trends in incidence, prevalence, and survival of patients on kidney replacement therapy (KRT) for end-stage kidney disease (ESKD) in Europe from 2008 to 2017. The incidence of KRT remained stable initially but increased slightly in recent years, primarily driven by an increase in older male patients. Diabetes mellitus was found to be a leading cause of kidney failure. The prevalence of KRT increased over the study period, and patient survival on KRT improved.

Use of regional citrate anticoagulation with medium cut-off membrane: pilot report

Summary: This study assessed the feasibility of regional citrate anticoagulation in expanded hemodialysis. The results showed that the standard citrate protocol could be successfully adopted for use in expanded hemodialysis with a medium cut-off membrane. Electrolyte and acid-base balances were relatively well-controlled and anticoagulation effectiveness was excellent.
