Simultaneous changes in the calcium-sensing receptor and the vitamin D receptor under the influence of calcium and calcitriol

Background. The regulatory mechanisms of parathyroid hormone (PTH) synthesis are complex, involving calcium, calcitriol, the calcium-sensing receptor (CaR) and the vitamin D receptor (VDR). In this study, the effects of calcium and calcitriol on the simultaneous expression of CaR and VDR mRNA and protein levels were assessed in parathyroid glands cultured in vitro. Methods. Parathyroid glands (N = 424) were removed and cultured for 24 h to study the effect of calcium on the CaR, VDR and PTH. In addition, the effect of calcitriol at low calcium concentrations (0.6 mM) on CaR and VDR levels was studied after 48 h of incubation. CaR, VDR and PTH mRNAs were measured by quantitative real-time PCR (qRT-PCR), and CaR and VDR protein levels were measured by immunohistochemistry. Results. PTH gene expression was reduced by high calcium concentration. No differences were found in the CaR mRNA levels among the different calcium concentrations tested (0.6 mM calcium: 100%; 1.2 mM calcium: 120%; 2.0 mM calcium: 112%; median values), but VDR gene expression rose when calcium increased (0.6 mM calcium: 100%; 1.2 mM calcium: 164%; 2.0 mM calcium: 195%; median values). Calcitriol increased both CaR (control: 100%; 10(-8) M calcitriol: 196%; median values) and VDR genes expression (control: 100%; 10(-8) M calcitriol: 176%; median values). The same findings were corroborated at protein levels for both CaR and VDR. Conclusions. In parathyroid glands cultured in vitro, calcium up-regulates VDR but not CaR. Conversely, calcitriol up-regulates both VDR and CaR mRNAs and protein levels, even at low calcium concentrations.

Summary: Chronic kidney disease (CKD) affects an estimated 850 million people worldwide and the leading causes are diabetes and hypertension. CKD can progress to end-stage kidney disease, requiring kidney replacement therapy. In addition, CKD is a risk factor for cardiovascular disease. Previously, angiotensin-converting enzyme inhibitors (ACEIs) and angiotensin receptor blockers (ARBs) were the mainstay of treatment, but they did not reduce cardiovascular events and mortality in CKD. However, sodium-glucose cotransporter-2 inhibitors (SGLT2i) have shown cardiovascular and renal benefits in clinical trials, revolutionizing cardiorenal protection for patients with diabetes. Recent trials have demonstrated the benefits of SGLT2i in reducing heart failure and progression to kidney failure in patients with CKD and/or heart failure. Specialty societies' guidelines are adapting to support wider use of SGLT2i. The consensus paper from EURECA-m and ERBP summarizes the latest evidence and guidelines for using SGLT2i for cardiorenal protection in CKD patients.

Vitamin D Receptor From VSMCs Regulates Vascular Calcification During CKD: A Potential Role for miR-145a

Summary: This study investigated the role of local vitamin D signaling in vascular smooth muscle cells (VSMCs) during chronic kidney disease (CKD)-induced vascular calcification (VC). The results showed that CKD-affected patients and mice with CKD exhibited increased VC and arterial expression of vitamin D receptor (VDR). Conditional gene silencing of VDR in VSMCs led to a significant decrease in VC in the mouse model of CKD. In addition, miR-145a was found to be involved in VC, and its expression was recovered in animals with deletion of VDR in VSMCs.

Summary: The objective of this study was to investigate the effect of high phosphorus exposure on the levels of miR-145 and its effect on vascular smooth muscle cell changes. These findings suggest that miR-145 could serve as an early biomarker of vascular calcification.

Smoking is identified as a risk factor for hyperkalemia in men, while hyperkalemia is associated with an increased risk of cardiovascular events in men. Summary: This study investigated the role of single nucleotide polymorphisms (SNPs) of the matrix gamma-carboxy glutamic acid protein (MGP) gene on atherosclerosis progression and cardiovascular events (CVE) in CKD patients. The results showed that the rs1800802 polymorphism of MGP is associated with increased risk of plaque progression and CVE in CKD patients.
