Recent developments in functional neuroimaging have provided a number of new tools for assessing patients who clinically appear to be in a vegetative state. These techniques have been able to reveal awareness and even allow rudimentary communication in some patients who remain entirely behaviourally non-responsive. The implications of these results extend well beyond the immediate clinical and scientific findings to influencing legal proceedings, raising new ethical questions about the withdrawal of nutrition and hydration and providing new options for patients and families in that decision-making process. The findings have also motivated significant public discourse about the role of neuroscience research in society.

Neuroimaging and electrophysiological assessments may serve as more sensitive biomarkers to identify changes after tDCS, and future research should focus on integrating tDCS with rehabilitation programs to explore potential synergies.

Disruptions in Effective Connectivity within and between Default Mode Network and Anterior Forebrain Mesocircuit in Prolonged Disorders of Consciousness

Summary: New research suggests that prolonged disorders of consciousness (PDOC) are caused by structural and functional impairments in key cortical and subcortical networks, including the default mode network (DMN) and the anterior forebrain mesocircuit (AFM). Although the specific mechanisms remain unknown, disruptions in the striatal-pallidal pathway are believed to play a role in PDOC. Analysis of rs-fMRI data indicates that PDOC patients exhibit reduced coupling within the AFM, resulting in disturbances in the DMN.

Summary: Research using tDCS intervention in long-term PDOC patients successfully altered dynamic coupling between brain regions. The results indicate that M1-tDCS and cb-tDCS can improve patients' motor deficits, enhancing rehabilitation options and quality of life.

Applications of Advanced MRI to Disorders of Consciousness

Summary: Disorders of consciousness (DoC) after severe brain injury are challenging for clinicians due to uncertainties in diagnosis, prognosis, and management. Advanced MRI technology, including diffusion MRI and functional MRI, has the potential to improve understanding of DoC pathophysiology, aid in identifying patient consciousness, and enhance clinical prognostication. This article reviews emerging evidence for the application of advanced MRI in patients with DoC.

Functional Neuroimaging as an Assessment Tool in Critically Ill Patients

Summary: This study aimed to investigate the efficacy of functional neuroimaging in assessing cognitive function in acute disorders of consciousness. The results showed that functional neuroimaging could detect preserved auditory function and language comprehension, and even reveal covert conscious awareness in patients. Furthermore, there was a positive relationship between fMRI responsivity and the level of functional recovery.

Characterising stationary and dynamic effective connectivity changes in the motor network during and after tDCS

Summary: This study explored the effects of tDCS on the motor network in the brain and found that it has widespread effects on connectivity, extending beyond the targeted area and modulating connectivity between cortex, thalamus, and cerebellum. The changes have unique nonlinear temporal patterns. This research provides insights into the network-level effects of tDCS and may guide future optimization of its cognitive and clinical applications.

Use of functional magnetic resonance imaging to assess cognition and consciousness in severe Guillain-Barre syndrome

Summary: The study evaluated the potential of using functional neuroimaging for communication in patients with Guillain-Barre Syndrome. This method could be used for patients to express their wishes or preferences and assent to previously made decisions.
