Summary: This study investigated the common causes, routine diagnostic and therapeutic methods of acute pancreatitis in children in a pediatric gastrointestinal referral center. There were variations in causes and methods among different centers. It is important to consider the association between acute pancreatitis and systemic and metabolic diseases as well as the use of anticonvulsant drugs. The risks of unnecessary CT scans and inappropriate antibiotic prescription should be emphasized.

Summary: The review of 11 randomized trials with high-quality evidence showed that early cholecystectomy has definite advantages in reducing recurrent pancreaticobiliary events and pancreatitis in patients with ABP. However, more RCTs are needed to study the role of early cholecystectomy in patients with moderately-severe and severe ABP.

Summary: This study retrospectively investigated the occurrence of benign biliary strictures (BBS) in patients with acute biliary pancreatitis (ABP). The results showed that the majority of patients showed improvement in the stricture within 3 months after endoscopic management, and temporary stenting prevented complications during this period.

Summary: We propose a robust AP identification and diagnosis (RAPIDx) method by analyzing the proteomic fingerprints of nanoscale extracellular vesicles (EVs) from clinical samples. We track the circulating biological nano-particles (EVs) released by cells through bottom-up proteomics and establish close phenotype connections between EVs, cell types, and multiple tissues based on their specific proteomes. We identify serum amyloid A (SAA) proteins on EVs as potential biomarkers differentially expressed in AP patients. The RAPIDx platform enables the timely diagnosis and treatment of AP and promotes precision diagnostics and early diagnosis of pancreatic cancer.

Summary: This study aimed to evaluate abdominal CT features of acute biliary pancreatitis and to predict the biliary origin of a first episode of AP using a combination of CT and clinical data. The findings showed that age, alanine aminotransferase level, gallbladder gallstone, choledochal ring sign, liver spontaneous attenuation, and duodenal thickening were independently associated with ABP. The nomogram combining clinical and CT features demonstrated good diagnostic performance for early diagnosis of ABP.

Summary: The incidence of chronic pancreatitis is increasing, and its treatment involves nutrition optimization, pain management, and endoscopic and surgical interventions. This systematic review provides evidence-based recommendations on the etiology, diagnosis, and management of chronic pancreatitis and its complications, emphasizing the importance of behavioral modification, endoscopic measures, and surgery for pain control and management of endocrine and exocrine insufficiency.

Summary: Daily, large ingestions of soymilk can potentially lead to acute pancreatitis due to the trypsin inhibitor in soybean, which reduces trypsin activity and can cause inappropriate increase in intrapancreatic trypsin secretion when intestinal proteolytic activity decreases, exceeding the capacity of pancreatic secretory trypsin inhibitor.

Summary: Acute pancreatitis is a dangerous inflammatory disease of the pancreas with a high mortality rate. It is primarily caused by gallstone migration and alcohol abuse. The activation of trypsinogen, infiltration of inflammatory cells, and destruction of secretory cells are the main characteristics of this disease. However, there are controversies and uncertainties regarding the diagnostic and therapeutic methods due to incomplete understanding of its pathophysiology.

Summary: The timing of cholecystectomy has a significant impact on the outcomes of patients with moderately severe and severe ABP. Early cholecystectomy is associated with higher postoperative mortality and morbidity. Elderly and frail patients with severe complications of ABP are unlikely to benefit from early cholecystectomy.

Summary: In the USA, most patients with mild gallstone pancreatitis do not undergo cholecystectomy during the same hospitalization. This approach leads to higher recurrence of pancreatitis, increased mortality during readmission, and an additional $4.85 million/year in hospital costs nationwide.

Summary: This study evaluated inflammatory biomarkers, oxidative stress, and plasma metabolome in patients with biliary acute pancreatitis to improve prognosis. Hepcidin, CRP, and SIRI were found to be accurate markers of AP severity. Oxidative stress was found to play a key role in the early phase of AP, with various metabolites showing significant changes in AP severity. Hepcidin and SIRI were confirmed as new prognostic biomarkers, indicating the role of oxidative stress in the onset and progression of AP.

Summary: Biliary lithiasis is a global disorder that affects 20% of the world's population, often without symptoms. It can lead to acute cholecystitis when gallbladder stones move into the common bile duct. The severity of the disease varies, with mild cases treatable with oral antibiotics and severe cases requiring specific treatment like surgery.

Summary: A randomized trial conducted in European hospitals found that minimally invasive total gastrectomy after neoadjuvant therapy is not inferior to open total gastrectomy in terms of oncological quality of resection in Western patients with resectable gastric cancer. Furthermore, there were no differences in postoperative complications and recovery between the two surgical approaches.

Summary: Impaired LA reservoir and conduit strain are present in AF patients with extensive atrial fibrosis. Patients with a high degree of LA fibrosis had lower reservoir strain and conduit strain compared to patients with a low degree of LA fibrosis, while no difference was found for LA contractile strain. Future studies are needed to examine the biologic nature of this association and possible therapeutic implications.
