Mild behavioral impairment as a potential marker of predementia risk states in motor neuron diseases

Summary: This study aimed to explore mild behavioral impairment (MBI) in motor neuron diseases (MNDs) and evaluate its relationship with cognitive features. The results showed that at disease onset, the most impaired MBI domain was affective/emotional dysregulation, followed by impulse dyscontrol, apathy, and social inappropriateness. Greater MBI symptoms at onset were significant predictors of more severe longitudinal cognitive decline.

Reduction in grey matter atrophy in patients with relapsing multiple sclerosis following treatment with cladribine tablets

Summary: This study found that treatment with cladribine 3.5 mg/kg can significantly reduce grey matter (GM) atrophy in patients with relapsing multiple sclerosis (MS) compared to placebo, after a short period of pseudoatrophy. The study also suggests that GM damage is relevant in MS and may have important implications for physical and cognitive disability progression.

Multicenter data harmonization for regional brain atrophy and application in multiple sclerosis

Summary: In patients with multiple sclerosis, the determination of brain atrophy in specific regions is clinically relevant. However, analyzing large datasets is challenging due to the increased variability in multicenter data. This study compared different methods to correct for center effects and investigated regional gray matter volume in patients with relapsing-remitting multiple sclerosis using a large multicenter dataset. The results showed that harmonization based on subsampling effectively reduced the residuals of the statistical model applied, and the multicenter results were more robust compared to findings from single-center analysis, highlighting the importance of data repositories from multiple centers.

Summary: This study identifies different subgroups of patients with secondary progressive multiple sclerosis (SPMS) based on their longitudinal trajectories of disability. The findings suggest that SPMS patients progress at greatly different rates and the identification of distinct trajectories can guide better patient selection in future clinical trials. Additionally, the distinct trajectories may reflect heterogeneous pathological mechanisms of progression.

Summary: The current designs for clinical trials treating progressive multiple sclerosis (MS) have limitations and have not been successful. This paper discusses complex innovative trial designs and intermediate and composite outcomes to improve trial design efficiency in MS. Recommendations include increasing the use of complex innovative designs, developing biomarkers to enrich trial populations, prioritizing intermediate outcomes targeting therapeutic mechanisms, and investigating data linkage for long-term outcomes. Novel trial designs and further development of intermediate outcomes may improve clinical trial efficiency and address novel therapeutic questions in MS.
