Summary: This study evaluated the effect of rehabilitation in preventing decreased functional status in elderly patients after community-acquired pneumonia. The results showed that aspiration and pneumonia severity index played important roles in reducing functional status, while the effect of rehabilitation in preventing functional status decline remains unclear.

Summary: In a trial evaluating the efficacy of hydrocortisone in treating community-acquired pneumonia, researchers found a nearly 50% lower 28-day mortality rate in the hydrocortisone group compared to the placebo group. They also observed a higher incidence of septic shock in the placebo group. The trial did not allow the use of open-label glucocorticoids, which may have contributed to the higher mortality rate in the placebo group.

Summary: This study aimed to evaluate clinically significant sex differences that could have an effect on the choice of treatment and outcomes of urinary tract infection (UTI) in aged 80 and over hospitalized patients with community-acquired UTI. The study found that male patients had a higher rate of septic shock at admission and higher rates of indwelling urinary catheter, while female patients were more likely to have cognitive impairment. There were no differences in outcomes between the sexes.

Summary: This study constructed a nomogram to predict 28-day mortality in elderly patients with SCAP by analyzing clinical parameters. The nomogram includes age, Glasgow score, blood platelet, and blood urea nitrogen values. The AUC of the nomogram is 0.713, and the calibration curve for 28-day mortality shows high coherence between predicted and actual probability of mortality.

Summary: In this study, the association between sarcopenia index (SI) and the risk of septic shock and mortality in older patients with community-acquired pneumonia (CAP) was assessed. The results showed that SI effectively predicts mortality in older adults with CAP, while it was only found to be associated with septic shock in older females.

Summary: This study aimed to examine the association between beta-lactam plus doxycycline and mortality for patients hospitalized with community-acquired pneumonia. The study found that the use of doxycycline as part of guideline-concordant antibiotic therapy was associated with lower 30- and 90-day mortality than regimens without doxycycline.

Summary: Malnutrition, including undernutrition and obesity, has an impact on the risk and prognosis of adults with CAP. Undernutrition is associated with unfavorable clinical outcomes, while obesity is generally linked to higher survival rates in CAP patients. However, the etiology of CAP and the influenza A (H1N1) infection may influence these outcomes. Further larger-scale studies are needed to understand the physiological mechanisms behind these effects.

Summary: We examined the differences in gene expression profiles in patients with community-acquired pneumonia (CAP) who died during hospitalization and those who survived. Whole blood samples were obtained for genome-expression-profile analysis within 24 hours of admission. The study found significant differences in gene expression at hospital admission between patients who died and those who survived. These findings could help identify potential intervention targets and biomarkers for risk stratification.

Summary: Hospital-acquired pneumonia (HAP) in hospitalized middle-aged and elderly patients with schizophrenia is associated with older age, male gender, and longer hospitalization duration. Specific antipsychotic drugs such as quetiapine, clozapine, and olanzapine, as well as combination with aceglutamide, are risk factors for HAP infection. Minimizing the types and dosages of antipsychotic drugs while monitoring mental symptoms is important in preventing HAP in this population.

Summary: An observational study investigated the clinical characteristics and outcomes of octogenarians with COVID-19. Most patients presented with fever, dyspnea, and cough, with the majority having moderate illness severity according to WHO criteria. Laboratory analysis showed increased inflammatory parameters and abnormal lymphocyte counts. Treatment commonly included corticosteroids and biological therapies, but mortality rate remained high.

Summary: This study compared the efficacy of ceftriaxone and ampicillin in the treatment of hospitalized adults with community-acquired pneumonia (CAP). The results showed no significant difference in 30-day all-cause mortality between the two treatment groups, but the ampicillin group had a significantly lower rate of Clostridioides difficile infection (CDI). Further research is needed to confirm these results.

Summary: The study aimed to investigate the diagnostic value of adding oxygenation parameters to CURB-65 in predicting mortality in CAP patients. It was found that using CURBS-65 scoring instead of CURB-65 scoring may be more useful in predicting mortality.

Summary: The study revealed an overall reduction of MDR/PNS isolates in adults after the introduction of pneumococcal conjugate vaccines. However, a significant proportion of current resistant isolates are not covered by any of the upcoming PCV15/PCV20 vaccines. The burden of resistant disease is related to older patients with underlying conditions and caused by two major clones. Data show that MDR is not statistically significant in relation to increased mortality.

Summary: This study evaluated the healthcare costs related to post-stroke oropharyngeal dysphagia (OD) and its complications, finding that patients with OD incurred higher costs during hospitalization, at 3-month follow-up, and at 12-month follow-up. Additionally, the development of malnutrition and respiratory infections significantly increased long-term costs for patients with OD.

Summary: This study investigated the seroprevalence of SARS-CoV-2 antibodies among 2455 healthcare workers in June and October 2020, and April and November 2021. The results showed that the seropositivity for SARS-CoV-2 increased from 11.8% in June 2020 to 28.4% in November 2021. Close contact with COVID-19 cases without protection and working in the frontline were identified as the main risk factors. Additionally, vaccination led to a decrease in antibody levels, and some vaccinated individuals tested negative for spike protein antibodies in November 2021.

Summary: This study aimed to assess the efficiency of healthcare interventions in post-stroke oropharyngeal dysphagia (PS-OD) management. The findings suggest that screening and diagnosis of PS-OD can adjust and reduce hospitalization costs, while OD management after thrombolysis may not significantly reduce hospitalization costs. Videofluoroscopy is found to be the most cost-effective screening method. Rehabilitation programs and home enteral nutrition are also shown to be cost-effective. However, further evaluation and research on novel strategies are still needed.

Summary: This study assessed socioeconomic and depression-related risk factors for frailty in depressed older adults. The results showed that age, gender, osteoarthritis, pain, depressive symptom severity, low educational level, and low-income level were risk factors for frailty. The study also found that the severity of depressive symptoms had the highest independent effect on frailty in depressed older adults.
