BACKGROUND AND OBJECTIVE: The most commonly used prognostic mortality indexes for pneumonia take into account several variables including comorbidities, physical examination results, and laboratory test results, as well as age. Other factors such as functional status are not included. The objective of this study was to know whether the preadmission functional status was related to 30-day mortality in old or very old patients who were hospitalized for pneumonia. PATIENTS AND METHOD: This was a prospective study including all patients who were hospitalized for pneumonia in the Acute Geriatric Unit of Hospital de Mataro, Barcelona. We calculated the Pneumonia Severity Index (PSI), preadmission and admission Barthel Index (BI), Charlson Comorbidity Index and Mini Nutritional Assessment (MNA). Patients were assessed during hospitalisation and until death or 30 days after admission. RESULTS: We studied 117 patients, 69 (59%) were men. The mean age (standard deviation) was 84.7 (6.5) years. The 30-day mortality was 16.2%. The PSI score was 134.2 (31.8) on admission, and the BI scores on preadmission and admission were 60.3 (35.8) and 37.1 (33.5), respectively. In a multiple logistic regression model, using all variables statistically significant in the univariate analysis, those independently associated with 30-day mortality were: preadmission BI lower than 60 points (odds ratio = 4.89; 95% confidence interval, 1.27-18.9) and lymphopenia (odds ratio = 7.11; 95% confidence interval, 1.7-30.2). CONCLUSIONS: In very old patients who were hospitalized for pneumonia, preadmission functional status was an independent predictor of mortality. Functional status should be included in the severity indices of pneumonia in this population.

Summary: The study revealed an overall reduction of MDR/PNS isolates in adults after the introduction of pneumococcal conjugate vaccines. However, a significant proportion of current resistant isolates are not covered by any of the upcoming PCV15/PCV20 vaccines. The burden of resistant disease is related to older patients with underlying conditions and caused by two major clones. Data show that MDR is not statistically significant in relation to increased mortality.

Healthcare costs of post-stroke oropharyngeal dysphagia and its complications: malnutrition and respiratory infections

Sergio Marin, Mateu Serra-Prat, Omar Ortega, Monica Audouard Fericgla, Jordi Valls, Elisabet Palomera, Ramon Cunillera, Ernest Palomeras, Josep Maria Ibanez, Pere Clave

Summary: This study evaluated the healthcare costs related to post-stroke oropharyngeal dysphagia (OD) and its complications, finding that patients with OD incurred higher costs during hospitalization, at 3-month follow-up, and at 12-month follow-up. Additionally, the development of malnutrition and respiratory infections significantly increased long-term costs for patients with OD.

Prevalence and risk factors of SARS-CoV-2 antibody responses among healthcare workers (June 2020-November 2021)

Maria Pilar Barrufet, Mateu Serra-Prat, Elisabet Palomera, Alicia Ruiz, Gemma Tapias, Noemi Montserrat, Nicolas Valladares, Francisco Javier Ruz, Mireia Bolivar-Prados, Pere Clave

Summary: This study investigated the seroprevalence of SARS-CoV-2 antibodies among 2455 healthcare workers in June and October 2020, and April and November 2021. The results showed that the seropositivity for SARS-CoV-2 increased from 11.8% in June 2020 to 28.4% in November 2021. Close contact with COVID-19 cases without protection and working in the frontline were identified as the main risk factors. Additionally, vaccination led to a decrease in antibody levels, and some vaccinated individuals tested negative for spike protein antibodies in November 2021.

Economic Evaluation of Clinical, Nutritional and Rehabilitation Interventions on Oropharyngeal Dysphagia after Stroke: A Systematic Review

Sergio Marin, Omar Ortega, Mateu Serra-Prat, Ester Valls, Laia Perez-Cordon, Pere Clave

Summary: This study aimed to assess the efficiency of healthcare interventions in post-stroke oropharyngeal dysphagia (PS-OD) management. The findings suggest that screening and diagnosis of PS-OD can adjust and reduce hospitalization costs, while OD management after thrombolysis may not significantly reduce hospitalization costs. Videofluoroscopy is found to be the most cost-effective screening method. Rehabilitation programs and home enteral nutrition are also shown to be cost-effective. However, further evaluation and research on novel strategies are still needed.

Summary: This study assessed socioeconomic and depression-related risk factors for frailty in depressed older adults. The results showed that age, gender, osteoarthritis, pain, depressive symptom severity, low educational level, and low-income level were risk factors for frailty. The study also found that the severity of depressive symptoms had the highest independent effect on frailty in depressed older adults.
