Environmental factors involved in axonal regeneration following spinal cord transection in rats

A recent study of a rat model treated with grafted collagen filament (CF) after spinal cord transection showed dramatic recovery of motor function but did not report on the acute-stage phenomenon. In the present study, we describe molecular and histological aspects of the axonal regeneration process during the acute stage following spinal cord transection. The spinal cord of 8-week-old rats was completely transected, and a scaffold of almost the same size as the resected portion was implanted in the gap. Changes in the mRNA expression of four neurotrophic factors [nerve growth factor (NGF), brain-derived neurotrophic factor (BDNF), NT-3, and glial cell-derived neurotrophic factor (GDNF)] were analyzed after 72 h. The expression of BDNF and NT-3 mRNA increased significantly in the CF-grafted group compared to the nongrafted group. Immunostaining for BDNF and NT-3 revealed that cells positive for these neurotrophic factors extended along the collagen filaments in the CF-grafted group. Similarly, astrocytes extended into the collagen filament scaffold together with the neurotrophic factors and partly across a border line. These findings indicate that collagen filament helps to reduce scar tissue, supports the expression of neurotrophic factors, and serves as a scaffold for the outgrowth of regenerating axons.

Current Concepts of Biomaterial Scaffolds and Regenerative Therapy for Spinal Cord Injury

Summary: Spinal cord injury (SCI) is a devastating condition that causes significant neurological impairment and imposes substantial social and financial burdens. Current management approaches focus on symptomatic treatment, lacking effective therapeutic strategies. However, recent advancements in regenerative medicine have shown promise in preclinical and clinical studies. The utilization of biomaterial scaffolds, combined with stem cell therapy or other regenerative therapies, holds great potential in accelerating the development of novel therapeutic strategies for SCI. This review summarizes the latest insights from preclinical and clinical studies utilizing biomaterial scaffolds for regenerative therapy and provides a comprehensive overview of the biomaterial strategies employed.

Summary: This nationwide multi-center study aimed to investigate the incidence and characteristics of blunt cerebrovascular injury (BCVI) in elderly Japanese patients with traumatic cervical spine injuries (CSI) without major bone injury. The study enrolled 1512 patients from 33 institutions and found that 3.5% of elderly patients had CSI complicated by BCVI. BCVI was more common in patients with head injury, severe neurological deficit, cervical fractures, and cervical dislocation. Mortality and brain infarction rates from BCVI were 0.13% and 0.40%, respectively.

Summary: For older patients with decreased reserve function, traumatic cervical spine injuries often result in early mortality. This study aimed to identify prognostic factors for early mortality in this patient population. The analysis included 1512 patients aged >= 65 years who were hospitalized for treatment of traumatic cervical spine injuries. The results showed that older age, male sex, cervical spine fracture, complete motor paralysis, and chronic kidney disease were risk factors for early mortality.

Summary: This study aimed to investigate changes over a 10-year period in the profile of cervical spine and spinal cord injuries among elderly individuals in Japan. The study found a significant increase in the number of patients and surgical cases, without any changes in patient demographics. Posterior surgery was the most common approach, with an increase in the use of instrumentation surgery with screws and a longer fusion range in recent years. Worsening neurological symptoms, particularly C5 palsy, increased significantly in the later period. However, there were no significant differences in perioperative, major, and other complications, including mortality.

Summary: The aim of this study is to identify factors associated with the outcome of laminoplasty for degenerative cervical spondylolisthesis (DCS). Multivariate logistic analysis revealed that the postoperative C2-7 angle during neck flexion was the major factor related to a favorable outcome for DCS. Slippage itself did not directly affect the outcome and postoperative cervical alignment. Caution is recommended for surgeons performing laminoplasty on patients with risk factors for postoperative excessive kyphotic C2-7 angle during flexion.

Summary: This study aimed to analyze the demographic characteristics and short-term outcomes of traumatic cervical spine injuries in patients with dementia. A total of 1512 elderly patients aged >= 65 years with traumatic cervical injuries were included, and 95 patients (6.3%) had dementia. The analysis showed that patients with dementia were older, predominantly female, had lower body mass index, higher frailty index, lower pre-injury activities of daily living, and more comorbidities compared to patients without dementia. Patients with dementia had poorer activities of daily living at 6 months and a higher incidence of dysphagia up to 6 months. Kaplan-Meier analysis also revealed a higher mortality rate in patients with dementia.

Summary: The study investigated the impact of early post-injury respiratory dysfunction on neurological and ambulatory ability recovery in elderly patients with cervical spinal cord injury and/or fractures. The respiratory dysfunction group had lower home discharge and ambulation rates, and higher rates of severe paralysis at discharge and follow-up. Patients with severe respiratory dysfunction had a poorer prognosis.

The results showed significant associations between age, sex, and PPT values at different body sites. Determining site-specific reference intervals of PPTs for Japanese individuals using a handheld algometer is important for pain research.

Summary: This study used a three-dimensional finite element model to perform mechanical analyses on thoracolumbar vertebral fractures with different spinal alignments. The results showed that using 6PS may be more beneficial than 4PS in reducing stresses on the fractured vertebrae and instrumentation, regardless of the spinal alignment.

Summary: This retrospective multicenter study aimed to compare the prognosis of elderly patients with cervical diffuse idiopathic skeletal hyperostosis (cDISH) related injuries to matched controls with and without fractures. The study found that patients with cDISH-related injuries with fractures did not have significantly different complications, ambulation outcomes, or paralysis severity compared to controls. However, patients with cDISH-related injuries without fractures had significantly poorer ambulation at discharge compared to controls. Logistic regression analysis identified complete paralysis and age as significant risk factors for mortality.
