Summary: Patients with lumbar ossification of the ligamentum flavum (OLF) may have a high risk of developing concomitant ossification of the entire spinal ligament, and the etiology is still unclear. This study investigated the propensity for spinal ligament ossification in asymptomatic individuals with lumbar OLF. The results showed that subjects with more lumbar OLF were more likely to have multilevel thoracic OLF and coexisting ossification of the posterior longitudinal ligament (OPLL).

Summary: This study investigated the association between dyslipidemia and ossification of the posterior longitudinal ligament (OPLL) and ossification of the ligamentum flavum (OLF). The results showed a higher proportion of dyslipidemia in OPLL/OLF patients, and an association between all types of ossification and dyslipidemia.

Summary: Diffuse Idiopathic Skeletal Hyperostosis (DISH) and Ossification of the Posterior Longitudinal Ligament (OPLL) are common disorders characterized by the ossification of spinal ligaments. The cause is unknown but genetic contribution is hypothesized. Many studies on genetics of ectopic calcification disorders have been performed, mainly on OPLL, but involvement of identified genes has not been confirmed in human cases. Common variants in several genes have been associated with OPLL, but associations lack definitive significance and evidence of functional significance is modest. A multifactorial aetiopathogenesis is suggested for DISH and OPLL, with a subset of cases showing stronger genetic component.

Summary: Ossification of the posterior longitudinal ligament (OPLL) is a heterotopic ossification that may cause spinal cord compression. Patients with OPLL often have complications related to ossification of other spinal ligaments, and OPLL is now considered part of ossification of the spinal ligaments (OSL). Animal models are needed to elucidate the pathophysiology of OSL and develop therapeutic strategies.

Summary: This study evaluated the rate of increase in thickness and cross-section area of spinal ligament ossification in patients with thoracic myelopathy. The results showed that being female, aging, and having a lower BMI predisposed patients to have faster ossification growth. The rate of ossification progression in the thoracic spine was significantly higher than that in the cervical spine.

Surgeons should carefully evaluate the severity of adhesion, muscle damage, and necessity of posterior corrective surgery, as well as consider the patient's comorbidities and potential postoperative complications.

Summary: This retrospective cohort study investigated the morphological changes in the sacroiliac (SI) joints in individuals with and without ossification of the posterior longitudinal ligament (OPLL). It was found that SI joint ankylosis occurred more frequently in patients with OPLL compared to those without. The presence of multilevel ossification in OPLL was associated with a higher rate of SI joint degeneration and ankylosis.

Summary: This study aimed to explore the biomechanical effects of cervical rotatory manipulation (CRM) on the spinal cord, dura matter, and nerve roots with ossification of the posterior longitudinal ligament (OPLL). The results showed that OPLL led to increased stress on the cervical spinal cord under static compression and during CRM. The dura matter and nerve roots also experienced higher stress during CRM.

Summary: Hyperbaric oxygen therapy (HBOT) is a potential therapeutic modality that has been recognized for its favorable mechanisms in various diseases, including sepsis-associated encephalopathy (SAE). HBOT has neuroprotective effects through its anti-inflammatory and antiapoptotic effects as well as increased tissue oxygenation capacity. However, there are caveats and limitations in applying HBOT in sepsis.
