PROTON RADIOTHERAPY FOR LIVER TUMORS: DOSIMETRIC ADVANTAGES OVER PHOTON PLANS

The purpose of the study is to dosimetrically investigate the advantages of proton radiotherapy over photon radiotherapy for liver tumors. The proton plan and the photon plan were designed using commercial treatment planning systems. The treatment target dose conformity and heterogeneity and dose-volume analyses of normal structures were compared between proton and photon radiotherapy for 9 patients with liver tumors. Proton radiotherapy delivered a more conformal target dose with slightly less homogeneity when compared with photon radiotherapy. Protons significantly reduced the fractional volume of liver receiving dose greater or equal to 30 Gy (V-30) and the mean liver dose. The stomach and duodenal V-45 were significantly lower with the use of proton radiotherapy. The V-40 and V-50 of the heart and the maximum spinal cord dose were also significantly lower with the use of proton radiotherapy. Protons were better able to spare one kidney completely and deliver less dose to one (generally the left) kidney than photons. The mean dose to the total body and most critical structures was significantly decreased using protons when compared to corresponding photon plans. In conclusion, our study suggests the dosimetric benefits of proton radiotherapy over photon radiotherapy. These dosimetric advantages of proton plans may permit further dose escalation with lower risk of complications. (C) 2008 American Association of Medical Dosimetrists.

The Effects of Compton Camera Data Acquisition and Readout Timing on PG Imaging for Proton Range Verification

Summary: The purpose of this study was to investigate how the characteristics of the data acquisition electronics of a Compton camera affect the quality of the recorded prompt gamma interaction data and reconstructed images during clinical proton beam delivery. The study used a simulation model to analyze the changes in recorded data as the number of data readout channels and the readout/reset timing were altered. The findings showed that increasing the number of readout channels and reducing the readout/reset timing resulted in a decrease in the number of recorded events and an improvement in data quality, allowing for higher quality image reconstruction even at high clinical dose rates.

Characterization of the Plastic Scintillator Detector System Exradin W2 in a High Dose Rate Flattening-Filter-Free Photon Beam

Summary: This study investigates the performance of the W2 scintillator compared to ion chambers for small field dosimetry. The results show excellent agreement between the two detectors in terms of beam profiles and percent depth dose curves. It is also demonstrated that the ion recombination effect of ion chambers can be adequately corrected using the two-voltage method.

Secondary Particle Interactions in a Compton Camera Designed for in vivo Range Verification of Proton Therapy

Summary: This study used Geant4 software to simulate the interactions of secondary particles in a Compton camera during the delivery of clinical proton beams. The results showed that for a 150-MeV proton beam, the majority of secondary particles reaching the camera modules were gamma rays, neutrons, and electrons, with a high percentage interacting with the modules. Strategies for reducing neutron backgrounds and non-PG radiation are needed for proton range verification using CCs.
