The purpose of this study was to investigate the relationship between liver fat fraction (FF) and diffusion parameters derived from the intravoxel incoherent motion (IVIM) model. Thirty-six subjects with suspected nonalcoholic fatty liver disease underwent diffusion-weighted magnetic resonance imaging with 10 b-values and spoiled gradient recalled echo imaging with six echoes for fat quantification. Correlations were measured between FF, transverse relaxivity (R2*), diffusivity (D) and perfusion fraction (1). The primary finding was that no significant correlation was obtained for D vs. FF on vs. FF. Significant correlations were obtained for D vs. R2* (r=-0.490, P=.002) and f vs. D (r=-0.458, P=.005). The conclusion is that hepatic steatosis does not affect measurement of perfusion or diffusion and therefore is unlikely to confound the use of apparent diffusivity to evaluate hepatic fibrosis. Imaging Features at the Periphery: Hemodynamics, Pathophysiology, and Effect on LI-RADS Categorization

Summary: Liver lesions exhibit various enhancement patterns at dynamic contrast-enhanced imaging, with LI-RADS utilizing enhancement kinetics for HCC diagnosis. Understanding and analyzing these spatial and temporal features is crucial for accurate characterization of liver masses. Focus is on patterns such as targetoid appearance, rim arterial phase hyperenhancement, and peripheral discontinuous nodular enhancement, which can aid in specific HCC diagnosis and treatment response assessment.

Summary: The major features of the Liver Imaging Reporting and Data System (LI-RADS) have different independent associations with hepatocellular carcinoma (HCC). Arterial phase hyperenhancement and washout have the strongest associations with HCC in CT/MRI, while threshold growth has no association. In CEUS images, APHE, late and mild washout, and a size of at least 20 mm are associated with HCC.

Summary: This study examined the dose-response effects of consuming high fructose corn syrup (HFCS)-sweetened beverages on hepatic lipid content and insulin sensitivity. The results showed that consuming HFCS-sweetened beverages was associated with an increase in hepatic lipid content and a decrease in insulin sensitivity. These findings strengthen the mechanistic evidence linking HFCS-sweetened beverage consumption to metabolic dysregulation, nonalcoholic fatty liver disease, and type 2 diabetes risk.

Monte Carlo modeling of hepatic steatosis based on stereology and spatial distribution of fat droplets

Jinyang Wang, Xiaoben Li, Mengyuan Ma, Changqing Wang, Claude B. Sirlin, Scott B. Reeder, Diego Hernando

Summary: This study aimed to model hepatic steatosis in adult humans with non-alcoholic fatty liver disease based on the stereology and spatial distribution of fat droplets. Histological analysis of liver biopsy specimens was performed, and the morphological features of fat droplets were characterized using the gamma distribution function in 2D and 3D spaces. Correlation analysis showed a strong relationship between the morphological features and fat fraction. Monte Carlo modeling was used to simulate the distribution of fat droplets in tissue. This study provides insights into the MRI signal behavior in the presence of liver steatosis.

LI-RADS: Looking Back, Looking Forward

Summary: Since its release in 2011, LI-RADS has evolved from a single algorithm for HCC diagnosis to a multialgorithm system covering all major liver imaging modalities. It has achieved global adoption in clinical care and research and unified HCC diagnostic systems in the US. However, there are still gaps in knowledge, such as challenges in surveillance, diagnostic population definition, and reporting. Future directions will focus on addressing these challenges and incorporating advanced technologies to create a probability-based system for liver cancer diagnosis and prognostication.

Hepatocellular carcinoma surveillance - utilization, barriers and the impact of changing aetiology

Daniel Q. Q. Huang, Amit G. Singal, Fasiha Kanwal, Pietro Lampertico, Maria Buti, Claude B. Sirlin, Mindie H. Nguyen, Rohit Loomba

Summary: In this review, the authors discuss the current status of hepatocellular carcinoma (HCC) surveillance and the challenges that remain. They highlight the changing causes of liver disease and the need for strategies to improve the utilization and quality of surveillance. The underutilization of surveillance is attributed to limited disease awareness, knowledge gaps, and difficulty in identifying high-risk individuals. Non-alcoholic fatty liver disease and alcohol-associated liver disease are emerging as significant causes of HCC-related death, posing unique barriers to surveillance. The sensitivity of current surveillance approaches is limited, especially for patients with cirrhosis or obesity.

Summary: The aim of this study was to investigate the variation of DCE-MRI-derived kinetic parameters in brain tumors as a function of acquisition time. The results showed that K-ep and V-e were time-dependent and required longer scan times to obtain reliable parameter values, while K-trans was time-independent and remained the same in all acquisition times, making it a reliable parameter for short acquisition times.

On the loss of image contrast in double-inversion-recovery prepared T2*MRI of Intramyocardial hemorrhage

Xingmin Guan, Xinheng Zhang, Hsin-Jung Yang, Rohan Dharmakumar

Summary: This study aims to investigate why DIR-prepared dark-blood T2* weighted images have lower SNR, CNR, and diagnostic accuracy for intramyocardial hemorrhage (IMH) detection compared to non-DIR-prepared bright-blood T2* images. Through phantom and animal studies, it was confirmed that the signal loss on DIR-prepared T2* images mainly originates from spin-relaxation during the DIR preparation. Therefore, when used for IMH detection, extra attention should be paid to the SNR of DIR-prepared dark-blood T2* imaging protocols.

Myelination of preterm brain networks at adolescence

Beatriz Laureano, Hassna Irzan, Helen OReilly, Sebastian Ourselin, Neil Marlow, Andrew Melbourne

Summary: Prematurity and preterm stressors have significant effects on the development of infants, especially at earlier gestations. While neonatal care advances have reduced preterm mortality rates, disability rates continue to grow in middle-income settings. Imaging the preterm brain using MR technology has improved our understanding of its development and the affected regions and networks. This research aims to support interventions, improve neurodevelopment, and provide accurate prognoses for preterm infants. This study focuses on the fully developed brain of extremely preterm subjects and examines myelin-related biomarkers to assess long-term effects. The findings suggest altered connectivity and cognitive outcomes in the adult preterm brain.

Compensation of concomitant field effects in double diffusion encoding by means of added oscillating gradients

Julian Rauch, Frederik B. Laun, Peter Bachert, Mark E. Ladd, Tristan A. Kuder

Summary: This study presents a method for reducing concomitant field effects in double diffusion encoding (DDE) sequences by adding oscillating gradient pulses. The modified sequences successfully reduced accumulated concomitant phase without significant changes in the original sequence characteristics. The proposed method led to an increase in signal-to-noise ratio (SNR) for phantom and in vivo experiments, supported by simulations.
