Hydroxychloroquine and pregnancy on lupus flares in Korean patients with systemic lupus erythematosus

We investigated the clinical and laboratory characteristics of pregnancies with systemic lupus erythematosus (SLE) and identified lupus flare predictors during pregnancy. Additionally, we examined lupus activity and pregnancy outcomes in SLE patients who continued, discontinued or underwent no hydroxychloroquine (HCQ) treatment during pregnancy. We retrospectively analyzed 179 pregnancies in 128 SLE patients at Seoul St. Mary's Hospital, Korea, between 1998 and 2012 and then assessed the clinical profiles and maternal and fetal outcomes. Overall, 90.5% of pregnancies resulted in a successful delivery and were divided into two groups: those who experienced lupus flares (80 pregnancies, 44.7%) and those who did not (99 pregnancies, 55.3%). Increased preeclampsia, preterm births, low birth weight, intrauterine growth restriction (IUGR), and low 1-minute Apgar scores occurred in pregnancies with lupus flares compared to pregnancies in quiescent disease. Lupus flares were predicted by HCQ discontinuation, a history of lupus nephritis, high pre-pregnancy serum uric acid and low C4 levels. Our study indicates that achieving pre-pregnancy remission and continuing HCQ treatment during pregnancy are important for preventing lupus flares.

Influences of pregravid liver enzyme levels on the development of gestational diabetes mellitus

Summary: This study found that elevated pregravid gamma-glutamyl transferase and alanine aminotransferase levels were associated with an increased risk of gestational diabetes mellitus, especially in women with obesity or MetS. However, significant risks were also present in women without obesity or MetS. These results suggest that elevated pregravid liver enzyme levels significantly increase the risk of developing gestational diabetes.

Current status of pertussis vaccination during pregnancy and influencing factors in Korea

Summary: The study in Korea found that 67% of pregnant women received pertussis vaccination during pregnancy, but 27.9% did not receive information about pertussis and vaccination. Independent influencing factors for maternal pertussis vaccination included being informed, being informed by OBGYN doctors, and residing in metropolitan areas. Around 56.3% of maternity HCPs routinely recommended pertussis vaccination, with factors affecting routine recommendation being awareness of guidelines and personal pertussis vaccination history within 10 years.

Predictors of joint damage in patients with rheumatoid arthritis: focus on short- and long-term effects of intra-articular glucocorticoid injections

Association between pregravid liver enzyme levels and gestational diabetes in twin pregnancies: a secondary analysis of national cohort study

Summary: This study found that high levels of pregravid GGT and ALT were associated with an increased risk of GDM + IT in women pregnant with twins, regardless of pregravid obesity or metabolic syndrome. COVID-19 Vaccine Acceptance during Pregnancy and Influencing Factors in South Korea

Summary: Pregnant women were initially excluded from COVID-19 vaccination in South Korea, but their vaccine acceptance and influencing factors were unknown. This study found that receiving vaccine information from obstetrics and gynecology doctors, cohabitant COVID-19 vaccination, and being in the second trimester were independent factors influencing maternal COVID-19 vaccination. Concerns about the vaccine's impact on the fetus and distrust in vaccine effectiveness were the main reasons for vaccine hesitancy among women who did not want to get vaccinated.

Effects of Amnioreduction before Physical Examination-Indicated Cerclage on Pregnancy Outcomes: A Propensity Score Matched Study

Summary: This study examined the effects of amnioreduction on pregnancy outcomes in women undergoing cerclage operations. The results showed that amnioreduction before cerclage was associated with a shorter time to delivery, but also a higher risk of preterm birth and poor neonatal outcomes. Careful consideration is necessary when deciding to perform amnioreduction before cerclage.

Prediction of Pregnancy-Associated Hypertension Using a Scoring System: A Multicenter Cohort Study

Summary: This study developed a risk scoring model for pregnancy-associated hypertension (PAH) based on maternal pre-pregnancy characteristics. The model included mean arterial pressure (MAP) and pregnancy-associated plasma protein-A (PAPP-A) as predictive factors. The model showed superior performance in predicting PAH and preterm PAH compared to other models. Further studies are needed to validate this scoring model with biomarkers and uterine artery Doppler.

Comparison of remission criteria in patients with rheumatoid arthritis treated with biologic or targeted synthetic disease- modifying anti-rheumatic drugs: results from a nationwide registry

Summary: This study investigated and compared remission rates in rheumatoid arthritis (RA) patients treated with different types of biologic or targeted synthetic disease-modifying anti-rheumatic drugs (b/tsDMARDs) from 2013 to 2019. The results showed that using the clinical disease activity index (CDAI), simplified disease activity index (SDAI), or Boolean criteria to assess sustained remission was more stringent, but consistent with the DAS28-based criteria in RA patients treated with b/tsDMARDs.
