Summary: A systematic review comparing adult living donor liver transplantation (LDLT) and deceased donor liver transplantation (DDLT) found that LDLT is associated with improved patient survival, reduced waiting time, and lower MELD score, despite an increased risk of biliary complications. Hepatic artery thrombosis rates did not differ between the two types of transplantation.

Summary: Myosteatosis is an important prognostic marker in patients undergoing deceased donor liver transplantation, particularly in the early post-operative phase. Low skeletal muscle mass alone is not significantly associated with graft and patient survival. Preoperative myosteatosis may be crucial for predicting outcomes in OLT recipients.

Summary: The study found that in the early postoperative period of kidney transplantation, large volume fluid therapy did not promote renal recovery but increased the risk of complications, particularly cardiopulmonary and gastrointestinal complications. Cardiovascular disease and high BMI were identified as strong risk factors for cardiopulmonary complications.

Summary: This study investigated the impact of NLR change on graft survival in living donor liver transplantation, finding that a decrease in NLR was associated with higher graft failure rates. Factors associated with NLR decrease included preoperative NLR, MELD score, intraoperative inotropic infusion and operative duration. Further research is needed to confirm these findings.

Summary: The aim of this study was to assess the association between graft-to-recipient weight ratio (GRWR) and hepatocellular carcinoma (HCC) recurrence in adult-to-adult living donor liver transplantation (LDLT). Three studies were identified, involving a total of 782 patients. The results showed that GRWR < 0.8% was associated with lower survival rates and higher tumor recurrence rates in HCC recipients, particularly for patients with HCC outside the established criteria. Considering individual GRWR as a contributor to HCC recurrence in LDLT patients may be beneficial.

Summary: Patients experiencing early graft loss (EGL) have lower retransplantation rates and inferior patient survival. High Kidney Donor Profile Index (KDPI) and donation after circulatory death (DCD) do not increase the risk of EGL or primary nonfunction (PNF) unless combined with prolonged cold ischemic time (CIT).

Summary: This study compared the performance of traditional statistical models and machine learning approaches in predicting survival following liver transplantation. The results showed that machine learning methods such as random forest performed better than traditional statistical models and previously reported survival scores. The study also identified major predictors associated with survival. This suggests that artificial intelligence has significant potential in aiding clinical decision-making during liver transplantation.

Summary: This study aimed to explore the long-term outcomes of living donor liver transplantation (LDLT) versus deceased donor liver transplantation (DDLT) for hepatocellular carcinoma (HCC) and predictors of recurrence after transplantation. The findings showed that there was no significant difference in overall survival (OS) between LDLT and DDLT, but LDLT provided better survival benefits in regions with low deceased organ availability. Milan criteria, level of alpha-fetoprotein (AFP), presence of vascular invasion, and tumor differentiation were significant predictors of recurrence.

Summary: By using a modified Delphi method, renowned international expert liver surgeons reached global consensus on the definition of TOLS in minimally invasive and open liver resection. This study provides the first international expert consensus-based definition of TOLS for assessing postoperative outcomes in liver surgery. The TOLS definition includes absence of intraoperative grade = 2 incidents, postoperative bile leakage grade B/C, postoperative liver failure grade B/C, 90-day major postoperative complications, 90-day readmission due to surgery-related major complications, 90-day/in-hospital mortality, and presence of R0 resection margin. This definition can be used to evaluate hospital performance and compare patient outcomes internationally.

Summary: This study aimed to develop and validate a predictive model for microvascular invasion (MVI) and create an online calculator for preoperative assessment in hepatocellular carcinoma (HCC) patients. The study found that alpha-fetoprotein, imaging tumor burden score, and neutrophil-to-lymphocyte ratio were associated with MVI. The developed online tool showed very good accuracy in predicting MVI and had important implications for prognostic stratification in HCC patients.

Summary: This study investigated the postoperative oncologic outcomes in patients with early and intermediate hepatocellular carcinoma (HCC) and found that patients with intermediate HCC were less likely to achieve textbook oncologic outcomes. Summary: This study examined the prognostic role of tumor burden score (TBS) relative to albumin-bilirubin (ALBI) grade among patients undergoing curative-intent resection of intrahepatic cholangiocarcinoma (ICC). The results showed that decreased tumor burden was associated with better long-term outcomes. Patients with high TBS and high ALBI grade had worse overall survival and higher risk of early recurrence and death.

Summary: This study aimed to identify features that could indicate the risk of pancreas allograft rejection. 74 features were collected from 79 patients who underwent simultaneous pancreas-kidney transplantation, and predictive models were built using the Naive Bayesian Classifier and Support Vector Machine. The results showed that the classifiers had good predictive performance, with AUROC and classification accuracy values ranging from 0.96 to 0.97 and 0.87 to 0.94, respectively. These findings suggest that successful classifiers can be developed for graft rejection prediction, supporting clinical decision making.

Summary: This study evaluated the impact of the donor risk index (DRI), recipient, and perioperative factors on post liver transplant outcomes. The DRI was not found to be associated with 1-year graft failure, but higher SOFA score or lactate upon ICU admission post LT were independently associated with higher odds of 1-year graft failure.
