Summary: This retrospective study evaluated 176 patients with blunt liver injury, finding that peripheral bile duct injuries can be treated conservatively while central bile duct injuries require surgical treatment. The management of hepatic artery pseudoaneurysms is dependent on the grade of injury, with severe cases (grade III-V) potentially requiring transcatheter arterial embolization (TAE).

Summary: A systematic review comparing adult living donor liver transplantation (LDLT) and deceased donor liver transplantation (DDLT) found that LDLT is associated with improved patient survival, reduced waiting time, and lower MELD score, despite an increased risk of biliary complications. Hepatic artery thrombosis rates did not differ between the two types of transplantation.

Summary: The study did not find a paradoxical protective effect of obesity on mortality after isolated severe blunt traumatic brain injury. Obese patients with isolated severe blunt traumatic brain injury are at increased risk of respiratory and venous thromboembolic complications, while underweight patients face the highest risks of morbidity and mortality after severe blunt traumatic brain injury.

Summary: Blunt thoracic aortic injury (BTAI) is a potentially fatal condition that requires prompt recognition and urgent management. Clinical manifestations of BTAI are not straightforward and may lead to misdiagnosis. The severity of aortic injury is crucial in determining mortality and morbidity rates, as well as guiding treatment decisions, especially when other organs are also involved. Delayed endovascular repair has become the mainstay of treatment for hemodynamically stable patients, providing lower perioperative mortality and morbidity rates compared to open surgical repair. However, concerns exist regarding long-term surveillance and radiation exposure, especially in younger patients. This paper aims to provide an update on diagnostic modalities and treatment strategies for BTAI patients.

Summary: Stress hyperglycemia is common in trauma patients, but in the most severe blunt liver injuries, it may be absent. The study suggests that absence of stress hyperglycemia on hospital admission could be a sign of the most severe liver injury. Blood glucose level changes could potentially serve as an additional diagnostic criterion for grading liver injury.

Summary: This study aimed to explore the long-term outcomes of living donor liver transplantation (LDLT) versus deceased donor liver transplantation (DDLT) for hepatocellular carcinoma (HCC) and predictors of recurrence after transplantation. The findings showed that there was no significant difference in overall survival (OS) between LDLT and DDLT, but LDLT provided better survival benefits in regions with low deceased organ availability. Milan criteria, level of alpha-fetoprotein (AFP), presence of vascular invasion, and tumor differentiation were significant predictors of recurrence.

Summary: This article summarizes a management guideline for blunt liver and spleen injuries, which divides the guidelines into four sections: admission, procedures, set free, and aftercare. The guidelines suggest admitting critically ill patients to the intensive care unit and stable patients to the ward with activity restrictions. Treatment recommendations include transfusions, angioembolization, and operative exploration. Discharge is based on clinical condition, and follow-up imaging is only indicated for symptomatic patients.

Summary: The management and outcomes of intimal blunt thoracic aortic injuries (BTAIs) can be affected by the discordance in grading between different criteria. The Society for Vascular Surgery (SVS) and University of Washington (UW) criteria have shown differences in classifying BTAIs, leading to potential variations in treatment decisions and patient outcomes.

Summary: This study found that donor liver transaminase levels do not predict transplant outcomes. Liver transplantation from donors with elevated transaminases can be accepted and transplanted confidently when other factors are favorable. This finding is important for improving organ utilization decision-making and preventing unnecessary organ discard.

Summary: Results from this study showed that emergent or urgent TEVAR for patients with concomitant BTAI and TBI was not associated with delayed stroke, in-hospital mortality, or aortic-related mortality. The timing of TEVAR should be guided by individual patient factors rather than surgical timing.

Summary: This review assesses technical advancements in split liver transplantation, such as in-situ splitting, machine perfusion, and arterial reconstruction. The use of in-situ splitting reduces cold ischemia time, while machine perfusion shows potential for improving graft quality. Additionally, the use of interposition grafts during split liver transplantation must be carefully considered due to increased risk of hepatic artery thrombosis.

Summary: The study compared characteristics and outcomes of drug-induced liver injury (DILI) caused by paracetamol and non-paracetamol medications, particularly herbal and dietary supplements, in a single center over a period of 2009-2020. It found that patients with paracetamol-related DILI had higher 90-day transplant-free survival rates compared to those with non-paracetamol DILI, especially cases involving herbal and dietary supplements. The proportion of DILI cases linked to herbal and dietary supplements has been increasing since 2009.
