Summary: Higher age and previous HE are risk factors for post-TIPS HE. Lactulose monoprophylaxis has no prophylactic effect, while the combination of lactulose and rifaximin can prevent HE recurrence but not in patients without a history of HE.

Summary: This study found that alterations in gut dysbiosis were negatively associated with the occurrence and severity of post-TIPS HE, and improving the microbiota can help reduce the incidence of HE. Analysis of the gut microbiota can be used to screen suitable patients for TIPS and prevent the occurrence of HE.

Summary: Transjugular intrahepatic portosystemic shunt (TIPS) is an effective method for treating complications of portal hypertension, but post-TIPS hepatic encephalopathy is a major complication. Only a few drugs have been proven effective in preventing this complication, so careful selection of high-risk patients is necessary.

Summary: Hepatic encephalopathy is a form of brain dysfunction caused by liver insufficiency and/or portal-systemic shunting, with inconsistencies in its detection and management. Diagnosis of HE involves consideration of its phenotypes, risk factors, and differential diagnosis from other similar conditions.

Summary: The link between muscular alterations and hepatic encephalopathy (HE) has been extensively studied. The imbalance in ammonia metabolism and removal between muscles and the liver leads to the development of HE in patients with liver failure or porto-systemic shunts. This review focuses on the clinical burden, pathophysiological mechanisms, and management principles of muscular alterations in cirrhosis.

Summary: Hepatic encephalopathy is a condition associated with both chronic and acute liver disease, characterized by mental status changes and neuropsychiatric impairment. It can be challenging to identify in children, but careful assessment is crucial as symptom progression may indicate cerebral edema and systemic deterioration. Hyperammonemia may be present, but its degree does not correlate with clinical severity. Current treatment options include addressing the underlying cause and reducing high ammonia levels with medications or extracorporeal liver support.

Summary: The study aimed to establish the normative data of PHES in healthy Thai individuals, assess the prevalence of MHE, and validate the use of S-ANT1 in detecting MHE in cirrhotic patients. The results showed that MHE was present in 26.6% of cirrhotic patients. S-ANT1 had a moderate positive correlation with PHES.

Summary: Hepatic encephalopathy (HE) is a frequent and severe complication of liver disease that often goes undiagnosed due to a lack of understanding and consensus on diagnosis. Non-specific symptoms and comorbidities in cirrhotic patients make differential diagnosis challenging. Guidelines are provided to assist clinicians in diagnosing and treating HE. Liver transplantation may be indicated for HE, but its reversibility post-transplantation and the consequences in patients with other neurological disorders remain controversial.

Summary: This study aimed to investigate the diagnostic ability of serum S100B in discriminating the grade of hepatic encephalopathy (HE) and its correlation with other parameters. The results showed that serum S100B level was not useful for differentiating the severity of HE, but it could be affected by age, serum bilirubin, and creatinine levels and was associated with MELD scores. Additionally, serum S100B levels showed the possibility of predicting 1-year mortality in cirrhotic patients.

Summary: Hepatitis D virus (HDV) coinfection with hepatitis B increases the risk of liver cancer, cirrhosis, and death compared to hepatitis B monoinfection. Accurate estimates of HDV prevalence are needed to effectively identify coinfected individuals. Implementing double reflex testing, including anti-HDV testing and HDV RNA testing, is the most effective method to generate estimates and identify undiagnosed individuals. This strategy is feasible and requires a relatively low number of tests.

Summary: This study analyzed the long-term benefits and safety of oral anticoagulation therapy prescribed in emergency departments for elderly patients with atrial fibrillation, and detected sex-related differences. The results showed that ED anticoagulant prescription is safe and contributes to a reduction in mortality for elderly patients with atrial fibrillation. Summary: Infections caused by multidrug-resistant bacteria are a significant problem for transplant patients. This retrospective study evaluated the impact of an Antimicrobial Treatment Optimization Program (PROA) on multidrug-resistant bacterial colonization and infection after liver transplantation. The results showed that there was good adherence to the PROA, and infection was the most common cause of readmission in the first year after transplantation. The incidence of infections was similar before and after PROA implementation, but there was a decrease in the number of multidrug-resistant bacterial infections.

Summary: Non-alcoholic fatty liver disease (NAFLD) is a spectrum of disorders ranging from simple steatosis to non-alcoholic steatohepatitis (NASH). NAFLD could result from dysfunction in multiple pathways and various molecular triggers. NASH involves inflammatory and fibrotic processes. Additionally, NAFLD is closely associated with several extra-hepatic comorbidities. Due to the heterogeneity and multifactorial etiology of NAFLD, clinical trials for treatment have had limited success. A narrative review of the phenotypic characteristics of NAFLD patients and personalized treatment could lead to better outcomes.
