Epilepsy is a common neurological disease in tropical countries, particularly in sub-Saharan Africa. Previous work on epilepsy in sub-Saharan Africa has shown that many cases are severe, partly a result of some specific causes, that it carries a stigma, and that it is not adequately treated in many cases. Many studies on the epidemiology, aetiology, and management of epilepsy in sub-Saharan Africa have been reported in the past 10 years. The prevalence estimated from door-to-door studies is almost double that in Asia, Europe, and North America. The most commonly implicated risk factors are birth trauma, CNS infections, and traumatic brain injury. About 60% of patients with epilepsy receive no antiepileptic treatment, largely for economic and social reasons. Further epidemiological studies should be a priority to improve understanding of possible risk factors and thereby the prevention of epilepsy in Africa, and action should be taken to improve access to treatment.

Prevalence, determinants and prognostic value of high coronary artery calcium score in asymptomatic patients with diabetes: A systematic review and meta-analysis

Summary: This study systematically reviewed and meta-analyzed the prevalence, determinants, and prognostic value of coronary artery calcium score (CACS) in asymptomatic patients with diabetes, finding that age, male sex, non-black ethnicity, and diabetes duration were risk factors for high CACS. An increase in CACS was strongly associated with an increased risk for all-cause mortality and/or fatal and non-fatal cardiovascular events, with women potentially having a higher risk than men.

Knowledge of epilepsy, quality of life, and psychiatric comorbidities in Lebanese adults with epilepsy

Summary: This study examined the association between sociodemographic characteristics, clinical factors, knowledge of epilepsy, and the quality of life (QoL) and the presence of psychiatric comorbidities in Lebanese people with epilepsy. The results showed that good knowledge of epilepsy and the absence of psychiatric comorbidities are important predictors of good QoL in Lebanese people with epilepsy.

Costs of Dementia in Low- And Middle-Income Countries: A Systematic Review

Summary: The proportion of people living with dementia in LMICs is expected to reach 71% by 2050. The costs of dementia increase with the severity of the disease, with medical costs highest in the mild stage and social and informal care costs highest in the moderate and severe stages. Annual cost estimates per patient range from PPP$131.0 to PPP$66,143.6. Accurate data is needed to guide public health policies in LMICs.

The results indicated that the normal limits for ECG variables in this population differed slightly from those in hypertensive patients and emphasized the importance of considering ethnicity in ECG interpretation.

Hypertension and associated factors in HIV-infected patients receiving antiretroviral treatment in Burundi: a cross-sectional study

Summary: Currently, the life expectancy of people living with HIV is similar to the general population. Hypertension is a major public health issue in Africa and is often undiagnosed. The prevalence of hypertension among HIV-infected adults on ART in Burundi is 17.4%, and approximately 47.25% of HIV patients with hypertension are undiagnosed. Factors associated with hypertension include being overweight, obesity, longer duration of HIV infection, diabetes, and age.

Summary: This meta-analysis of data from 21 cohorts across six continents found that women have a higher risk of developing dementia than men, especially in low- and lower-middle-income economies. Longer education and former alcohol use are stronger risk factors for dementia in men than in women, while other risk factors have similar effects in both genders.

Summary: Based on individual participant data from 13 longitudinal ageing studies, the study found that social connections are associated with lower risks of mild cognitive impairment, dementia, and mortality. Specifically, good social connection structure and quality are linked to lower risk of incident mild cognitive impairment, while social connection structure and function are related to lower risk of incident dementia and mortality. In Asian cohorts, being married/in a relationship is associated with reduced risk of dementia, and having a confidante is associated with reduced risk of dementia and mortality.

Prevalence and associated factors with peripheral neuropathies in the general population in the rural area of Adjohoun in Benin

Summary: This study aimed to investigate the prevalence and associated factors of peripheral neuropathies (PN) in the general population of Adjohoun, Benin. The overall prevalence of PN was found to be 6.9%, with the main types being polyneuropathies (4.8%) and polyradiculoneuropathies (1.6%). Factors independently associated with PN included age greater than or equal to 40 years, diabetes, and hypertension. Taking action to reduce modifiable associated factors such as diabetes and hypertension could help reduce the occurrence of PN and their potential harmful consequences.

Effectiveness of a home-based telesurveillance program in reducing hospital readmissions in older patients with chronic disease: The eCOBAHLT randomized controlled trial

Summary: This study aimed to evaluate the feasibility and effectiveness of a 12-month remote monitoring program in preventing rehospitalizations in older patients with two or more chronic diseases returning home after hospitalization. The results showed that the risk of rehospitalization for chronic disease decompensation was significantly reduced in the intervention group receiving remote monitoring compared to the control group. The program combined home life technology, telecare, and biometric sensors.

Summary: This study aims to establish normal reference values for echocardiographic cardiac chambers dimensions and Doppler parameters in a Sub-Saharan African population. The results show that normal values in West Africa differ from those established in Caucasian populations, with greater left ventricular mass and wall thicknesses.

Potential teratogenicity of modafinil-Conflicting evidence, need for research

Summary: Central disorders of hypersomnolence include different types of narcolepsy, idiopathic hypersomnia, and hypersomnia associated with medical or mental disorders. Treatment options include non-pharmacological and pharmacological methods, but one of the first-line medications, modafinil, has been linked to a potential risk of birth defects when taken during organogenesis. Existing epidemiological studies have produced contradictory results, making it difficult to confirm or deny the risk. In clinical practice, it is recommended to suspend modafinil treatment during the first 10 weeks of pregnancy if possible. Further research is needed to clarify the teratogenic impact of modafinil.

Summary: The study found that the two ILAE classification schemes are broadly consistent, but the introduction of the combined onset seizure category in ILAE-2017 significantly reduces the proportion of mutually exclusive focal and generalized seizures. The comprehensive classification of etiology categories in ILAE-2017 will facilitate appropriate treatment and improve prognosis.
