Summary: Brain injury is a common complication of sickle cell anaemia (SCA), leading to reduced volume of white matter (WM) and grey matter (GM) structures in patients. This study examines the impact of silent cerebral infarction (SCI), vasculopathy, and anaemia on WM and regional GM volumes in African children. Results show that SCA patients have smaller volumes of WM, cortical and subcortical GM, as well as specific brain regions, compared to controls. SCI and vasculopathy further decrease subcortical GM volume, while SCA patients without SCI or vasculopathy exhibit larger putamen and hippocampus volumes. Haemoglobin levels only significantly affect GM volumes in controls. Overall, SCA patients generally have reduced GM volumes, although some subcortical regions may be spared. SCI and vasculopathy can impact changes in subcortical GM and WM volume, while anaemia may affect brain volume in non-SCA children.

Summary: Cerebrovascular injury is common in children with sickle cell anaemia (SCA) in sub-Saharan Africa, but limited access to MRI-MRA hinders detection. Blood-based biomarkers, including neurofilament light chain (NfL), may help identify cerebral infarcts. In a study with Ugandan children with SCA, NfL levels were significantly higher in those with MRI-detected infarcts and elevated TCD velocity. NfL levels could potentially be used as a biomarker for SCA cerebrovascular injury.

Summary: This study aimed to investigate the prevalence, severity, hospital outcome, and factors associated with acute kidney injury (AKI) in hospitalized children with sickle cell anemia (SCA). The results showed that AKI occurred in 27 out of 155 children with SCA (17.4%), with 33.3% reaching stage 3. Hepatomegaly, splenomegaly, dipstick proteinuria, and hematuria were more common in AKI patients. AKI patients had lower hematocrit and serum bicarbonate compared to those without AKI. AKI was also associated with a longer hospital stay.

The prevalence of supernormal GFR and decreased GFR in children with SCA was reported, with no significant association between clinical features of chronic kidney disease (CKD) and decreased GFR in the SCA group. Regular renal evaluation is recommended for early detection and treatment of renal complications in children with SCA.

Summary: The surface-exposed high mannose N-glycans on diseased or oxidized red blood cells bind to mannose receptor CD206 on phagocytes, mediating phagocytosis in sickle cell disease and malaria. This recognition of surface N-linked high mannose glycans as a response to cellular stress is a common molecular mechanism in the pathogenesis of sickle cell disease and resistance to severe malaria in individuals with sickle cell trait.

Summary: This study explored the association between novel urinary biomarkers and albumin-creatinine ratio (ACR) in adults with sickle cell anaemia. Among the participants, 13 (35.2%) had persistent albuminuria (PA). Urinary levels of clusterin, retinol-binding protein 4, alpha-1 microglobulin, and angiotensinogen were significantly higher in participants with PA compared to those without PA. Multivariable analysis revealed that urinary angiotensinogen was associated with ACR, suggesting its potential in identifying sickle cell anaemia patients at risk for kidney disease.

Summary: Hydroxyurea (HU) is beneficial in sickle cell anaemia (SCA) patients by increasing foetal haemoglobin (HbF), but the response varies. The role of miRNA in HbF regulation has been minimally investigated. In this study, miRNA expression and HbF induction were evaluated in relation to HU therapy. After 3 months of HU therapy, HbF levels increased significantly. Several miRNAs were up-regulated or down-regulated after HU therapy, and certain miRNAs were strongly associated with HbF induction. The study suggests that miR-210, miR16-1, miR-29a, and miR-96 play a role in regulating the gamma-globin gene and HbF induction.
