Several studies indicate a relationship between hypovitaminosis D, survival, vascular calcification and inflammation. In addition to its central role in the regulation of bone mineral metabolism, vitamin D also contributes to other systems, including the immune, cardiovascular and endocrine systems. Vitamin D analogs reduces proteinuria, in particular through suppression of the renin-angiotensin-aldosterone system (RAAS) and exerts anti-inflammatory and immunomodulatory effects. In particular vitamin D deficiency contribute to an inappropriately activated RAAS, as a mechanism for progression of chronic kidney disease (CKD) and/or cardiovascular disease. Human and sperimental models of CKD showed that vitamin D may interact with B and T lymphocytes and influence the phenotype and function of the antigen presenting cells and dendritic cells, promoting properties that favor the induction of tolerogenic T regulators rather than T effectory. Interstitial fibrosis may be prevented through vitamin D supplementation. Renal myofibroblast, an activated fibroblast with expression of a molecular hallmark alpha-smooth muscle actin (alpha-SMA), is generally considered the principal matrix-producing effector cells that are responsible for the excess production of extracellular matrix (ECM) components in the fibrotic tissues. It turns out that calcitriol effectively blocks myofibroblast activation from interstitial fibroblasts, as evidenced by suppression of TGF-beta 1-mediated alpha-SMA expression. Copyright (C) 2013 S. Optimizing the Outcomes of Percutaneous Coronary Intervention in Patients with Chronic Kidney Disease

Summary: The net benefit of percutaneous coronary intervention (PCI) in patients with chronic kidney disease (CKD) is not well established. However, proper management is crucial to improve outcomes for this patient population, including pre-procedural preparation, reducing contrast usage during the procedure, implementing modern strategies, and personalized antithrombotic therapy.

Summary: Copper is a crucial element for body homeostasis and its imbalances can lead to various diseases. Both copper deficiency and excessive exposure can cause harm to multiple organs. Particularly, the influence of copper on diabetes and its complications, such as Diabetic Kidney Disease, has been studied extensively.

Phosphate Control in Peritoneal Dialysis Patients: Issues, Solutions, and Open Questions

Summary: Hyperphosphatemia is a common complication in advanced chronic kidney disease and can lead to cardiovascular morbidity and mortality. This review focuses on the management of phosphatemia in uremic patients receiving peritoneal dialysis. The control of serum phosphate in peritoneal dialysis patients is influenced by various factors such as patient transporter characteristics, peritoneal dialysis modalities, prescriptions, and diet. There is a need for more clinical trials specifically designed for peritoneal dialysis patients to evaluate the effects of multiple therapeutic approaches on serum phosphate control and clinical outcomes.

How to Choose the Right Treatment for Membranous Nephropathy

Luigi Peritore, Vincenzo Labbozzetta, Veronica Maressa, Chiara Casuscelli, Giovanni Conti, Guido Gembillo, Domenico Santoro

Summary: Membranous nephropathy is a common autoimmune disease that affects the glomeruli and can lead to nephrotic syndrome and end-stage renal disease. The discovery of autoantibodies has improved our understanding of the disease and guided its diagnosis and treatment. Treatment options include supportive therapy and immunosuppressive drugs, but the choice of treatment should be based on individual patients.

Summary: Youth-onset T2DM is a significant global burden with high prevalence rates in China and the United States, driven by genetic factors and coexisting risk factors like obesity and hyperglycemia. Understanding the pathogenesis and treatment evolution of DKD in this population is crucial to reducing the risk of DKD progression.

Amyloidosis and Glomerular Diseases in Familial Mediterranean Fever

Rossella Siligato, Guido Gembillo, Vincenzo Calabrese, Giovanni Conti, Domenico Santoro

Summary: Familial Mediterranean fever is a genetic autoinflammatory disease that can lead to severe complications, with amyloid A renal deposition-induced amyloidosis being the most fearsome. Differential diagnosis of nephrotic syndrome among FMF patients is necessary, and early detection of microalbuminuria is a key signal of kidney impairment.

Vitamin D and Glomerulonephritis

Guido Gembillo, Rossella Siligato, Michela Amatruda, Giovanni Conti, Domenico Santoro

Summary: Vitamin D plays a crucial role in Chronic Kidney Disease patients by reducing proteinuria and slowing kidney disease progression. It has anti-inflammatory effects and protects podocytes, contributing to the inhibition of kidney inflammation.
