Tissue engineering is a multidimensional process combining cells, scaffold matrices, and chemical signals to produce a structure similar to a target tissue. These techniques have opened a completely new field in diagnosis and therapy in numerous fields, including that of laryngology. Laryngeal tissue engineering has emerged in the last decade, although clinical applications are rare. The reasons therefore are numerous including ethical reasons, as well as the extremely complex anatomical structure of the vocal fold. The search for new treatment options has also enlarged our knowledge about the microphysiology and micropathophysiology of the vocal fold. To date, only specific growth factors are in clinical use for treatment of vocal fold atrophy. Big advances have been made in creating state-of-the-art scaffolds with various techniques including biomaterials as well as fully synthetic polymers. These scaffolds are supposed to provide an optimal environment for residual or implanted cells. Several in vitro settings showed practicability of these scaffolds, also in studying effects of growth factors. Cell therapy is a powerful tool in regenerative medicine but bears the uncertainty of possible malignant transformation. The aim of this review was to give a comprehensive overview about current knowledge in the field of laryngeal tissue engineering and regenerative medicine, including restoration of both vocal folds and laryngeal cartilage, and furthermore to elucidate further trends in this fascinating field.

Laparoscopic Ladd Procedure for Malrotation in Newborns and Infants

Summary: This study reviewed the outcomes of laparoscopic surgery for neonate/infant malrotation and found it to be feasible and safe in hemodynamically stable patients, with a notable conversion rate and reintervention rate. The presence of volvulus was associated with symptom recurrence and the need for conversion during surgery. The commonly practiced approach involved laparoscopic Ladd's procedure with appendectomy and without cecopexy, which was associated with minor complications.

Esophagus stretch tests: Biomechanics for tissue engineering and possible implications on the outcome of esophageal atresia repairs performed under excessive tension

Summary: This study investigated light microscopic changes after uniaxial stretching of the ovine esophagus and found that esophageal stretching > 1.3 leads to tears in muscle fibers and rupture of capillaries. These findings may explain the decrease in microcirculation and scarring during repairs of esophageal atresias under excessive tension, providing insights into impaired motility in the esophagus.

Summary: Laparoscopic cholecystectomy in children is safe, although complications such as bile duct injury are a concern. Most complications are minor, but nearly 1/5 of cases require interventions, mainly due to bleeding and biliary tree complications.

Thoracoscopic Esophageal Atresia Repair: Outcomes Analysis Between Primary and Staged Procedures

Summary: This systematic review analyzed the outcomes of thoracoscopic repair of esophageal atresia (EA) between primary and staged repairs. Results showed that there were no significant differences in outcomes such as re-fistulation, anastomotic leaks, conversion rates, and mortality between primary and staged repairs. Successful repair with low complication rates was reported for both approaches.

Summary: This study reviewed outcomes in pediatric ACS/DAC after CDH repair, finding that ACS/DAC occur more frequently in neonates and a clear rationale behind the decision to perform DAC is lacking, with silo being the preferred approach. Criteria need to be established for DAC in CDH cases with large herniated content and small abdominal volume to prevent ACS.

Laparoscopic management of common cloaca: Current status

Summary: This study reviewed the literature on laparoscopic management of cloaca. The results showed that laparoscopic surgery could be used for repairing high rectum and performing urogenital separation, but there is currently no conclusive evidence to support that laparoscopic repair has better functional outcomes than open surgery.

Ultrastructural changes in esophageal tissue undergoing stretch tests with possible impact on tissue engineering and long gap esophageal repairs performed under tension

Summary: Esophageal biomechanical studies were conducted to investigate structural changes and obtain biomechanical values for tissue-engineered esophagus. Uniaxial stretching tests were performed on 1-month-old lamb esophagus samples. Results showed that stretching over 30% led to significant alterations in the intra- and extracellular structure, indicating the disruption of bonds. These findings are important for understanding structural changes in repaired esophageal atresias and for tissue engineering of the esophagus.

Iatrogenic Neonatal Esophageal Perforation: A European Multicentre Review on Management and Outcomes

Summary: The aim of this multicenter retrospective study and literature review was to review management and outcomes of neonatal esophageal perforation (NEP). The study found that NEP during NGT insertion is rare even in premature infants. In this small cohort, conservative management seems to be safe. A larger sample size will be necessary to answer questions on efficacy of antibiotics, antacids and NGT re-insertion time frame in NEP.

European Paediatric Surgeons' Association Consensus Statement on the Management of Neonatal Ovarian Simple Cysts

Summary: This study reviewed the literature on the management of neonatal ovarian simple cysts and formulated a consensus statement based on current evidence and peer practice. Although there is limited data on the potential benefits and complications of prenatal ultrasound-guided aspiration, neonates after such procedures should be followed for 6 months. Neonates with simple ovarian cysts larger than 4 cm should undergo surgical interventions within the first two weeks of life, with laparoscopic cyst aspiration and fenestration using bipolar instruments as the preferred approach. Ultrasound follow-up should be done at 3 months after surgical intervention and every 3 to 4 months with the conservative approach until 1 year.
