Summary: This study compares the short and intermediate-term outcomes of open repair and endovascular repair for descending thoracic and thoracoabdominal aortic aneurysms. The results show that open repair achieves similar in-hospital mortality and occurrence of paralysis and stroke compared to endovascular repair, but has better 10-year survival and lower aortic reintervention rate.

Summary: Non-elective fenestrated-branched endovascular aortic repair (FB-EVAR) for thoracoabdominal aortic aneurysms (TAAAs) is associated with higher early mortality and major adverse events (MAEs), increased all-cause mortality, and aortic-related mortality (ARM) compared to elective repair. Long-term follow-up is necessary to justify the treatment.

Summary: This study investigated the proportion of ruptured abdominal aortic aneurysms (AAAs) that were smaller than the current elective operative thresholds. It was found that 10% of AAA repairs were for patients with small AAAs. Patients with small ruptured AAAs were less likely to present with hypotension and were more likely to have undergone endovascular aneurysm repair (EVAR).

Summary: This study investigated the outcomes of thoracoabdominal aortic aneurysm (TAAA) patients with shaggy aorta (SA) treated with open surgical repair (OSR). The results showed that patients with SA had worse postoperative outcomes, including higher in-hospital mortality, postoperative acute renal failure, and postoperative embolization, compared to patients without SA. However, the grade of SA did not significantly impact postoperative outcomes.

Summary: Due to the lack of randomized controlled trials or controlled clinical trials, the safety and effectiveness of endovascular repair versus conventional open surgical repair in patients with thoracoabdominal aortic aneurysms cannot be determined, and there is no evidence available for the optimal surgical intervention for this group of patients. Conducting high-quality studies addressing this objective will be challenging due to logistical and ethical considerations for this life-threatening disease.

Summary: This study aimed to compare the outcomes of single or multistage approaches in fenestrated-branched endovascular aortic repair (FB-EVAR) for extensive thoracoabdominal aortic aneurysms (TAAAs). The study found that a multistage approach was associated with a decreased risk of mortality and/or permanent paraplegia at 30 days or within the hospital stay, and higher patient survival at 1 and 3 years compared to a single-stage approach.
