Effects of diabetes on small aortic aneurysms under surveillance according to a subgroup analysis from a randomized trial

Background: This study aims to investigate the impact of diabetes in the management of patients with small abdominal aortic aneurysms (AAA). Methods: Three-hundred sixty patients with small AAA (4.1-5.4 cm), enrolled in a randomized trial comparing early endovascular repair versus surveillance and delayed repair (after achievement of >5.5 cm or growth >1 cm/yr), were analyzed with standard survival methods to assess the relation between diabetes and risk of all-cause mortality, complications, and aneurysm growth (on computed tomography as per trial protocol) at 36 months. Baseline covariates were selected with partial likelihood stepwise method to investigate factors (demographic, morphologic, medications) associated with risk of aneurysm growth during surveillance. Results: Prevalence of diabetes was 13.6%. The hazard ratio (HR) for all-cause mortality at 36 months was higher in diabetic compared with nondiabetic patients: (HR, 7.39; 95% confidence interval [CI], 1.55-35.13; P = .012). Baseline aneurysm diameter was comparable between diabetic and nondiabetic patients enrolled in the surveillance arm and was related to subsequent aneurysm growth in covariance analyses adjusted for diabetes (49.3 mm for nondiabetic; 50.2 mm for diabetic). Cox analyses found diabetes as the strongest independent negative predictor of 63% lower probability of aneurysm growth >5 mm during surveillance (HR, 0.37; 95% CI, 0.15-0.92; P = .003). Kaplan-Meier cumulative probability of aneurysm growth >5 mm at 36 months was 40.8% in diabetics versus 85.1% in nondiabetics (HR, 0.32; 95% CI, 0.17-0.61). Conclusions: Progression of small AAA seems to be more than 60% lower in patients with diabetes. This may help to identify high-risk subgroups at higher likelihood of AAA enlargement, such as nondiabetics, for surveillance protocols in patients with small AAA. (J Vasc Surg 2012;56:1555-63.)

Summary: The paper describes a simple and helpful technique for challenging visceral vessel catheterization during complex aortic endovascular procedures, improving safety and ease of operation in demanding anatomies. By inflating a compliant balloon above the target arteries, it allows easy and safe advancement of the introducer sheath inside selected vessel, reducing shear forces and enhancing device pushability. This approach presents a useful, easy, and reproducible endovascular option that can be adopted for challenging vessel catheterization during advanced endovascular procedures.

Bridging stent graft fracture after branched endovascular aortic repair in a dual-center retrospective cohort study

Summary: This paper aims to assess the incidence and risk factors of bridging stent graft (BSG) fracture after thoracoabdominal aortic aneurysms branched endovascular aortic repair (BEVAR). The study found that BSG fracture is a rare but severe complication, not related to the type of stent used as bridging stent, but influenced by the use of multiple and different stents on the same vessel and the use of bare-metal stents for distal relining. Strict computed tomography angiography follow-up is recommended to detect target vessel instability.

Role of historical and procedural staging during elective fenestrated and branched endovascular treatment of extensive thoracoabdominal aortic aneurysms

Summary: For patients with extensive TAAAs treated with fenestrated/branched endovascular aortic repair, both historical and planned procedural staging were associated with reduced permanent SCI rates. However, no additional benefit was observed when procedural staging was performed in patients with historical staging and an intact collateral network. Further investigation is needed to explore the protective role of preoperative cerebrospinal fluid drainage placement.

Safety of Carotid Endarterectomy for Symptomatic Stenosis by Age: Meta-Analysis With Individual Patient Data

Summary: This study conducted a systematic review and meta-analysis of 47 studies and found that elderly patients with symptomatic carotid stenosis have a higher perioperative risk following carotid endarterectomy. The risk of stroke and death after surgery is significantly higher in octogenarians compared to nonoctogenarians. However, the perioperative stroke risk has declined in octogenarians over the past three decades. These findings suggest that selective urgent intervention should be considered for symptomatic elderly patients when medical therapy alone is ineffective.

Stent-Assisted Coil Embolization of a Saccular Visceral Aortic Aneurysm: Case Report and Review of the Literature
