Summary: This study developed a statistical model that takes into account various characteristics of AAA growth and applied it to predict AAA growth. The results demonstrated that the model accurately described the distribution of AAA growth and could be used to predict growth rates and the probability of exceeding a specific threshold for patients.

Summary: This study investigated the correlation between peak wall stress (PWS) and abdominal aortic aneurysm (AAA) geometry through computational simulations, using the Holzapfel model and varying orientations of collagen fibers and thicknesses of aortic layers. Material constants were estimated from biaxial tensile test results of normal abdominal aorta and AAA. Geometric factors such as aneurysm diameter, height, neck angle, and iliac angle were identified as factors affecting AAA rupture, with a simulation scenario created based on computed tomography data of AAA patients to estimate the correlation between PWS and AAA geometry.

Summary: Compared with the outcomes of the EVAR-1 trial, adherence to the EVAR-1 scan protocol in simulation models reduced AAA deaths and increased elective re-interventions. The European Society re-intervention guidelines provided the most clinically effective strategy, but with an increase in re-intervention rates. Annual ultrasound in primary care was the cheapest and most cost-effective strategy that reduced AAA-related deaths.

Summary: Previous studies have suggested that finite element analysis (FEA) can estimate the rupture risk of an abdominal aortic aneurysm (AAA); however, the value of biomechanical estimates over measurement of AAA diameter alone remains unclear. This study aimed to compare peak wall stress (PWS) and peak wall rupture index (PWRI) in participants with ruptured and asymptomatic intact AAAs. High aortic PWS had an inconsistent association with greater odds of aneurysm rupture in patients with a large AAA.

Summary: The study found that the clinical value of 3D-US for AAA surveillance is significantly higher than that of 2D-US. This new modality can substantially change clinical management for approximately one fourth of patients with AAA. Further research is needed to evaluate the clinical consequences of this paradigm shift in AAA diagnostics.

Summary: This study investigated the impact of a relative-size-index of the abdominal aortic diameter on the prevalence estimates of abdominal aortic dilatations compared to absolute diameters. Two gender-specific prediction equations were developed based on body-surface area and age. The results showed that the prevalence of abdominal aortic dilatations, abdominal aortic aneurysms, and repair-recommendable abdominal aortic aneurysms defined by absolute size and relative size index were significantly higher in males than females.

Summary: The paper describes a simple and helpful technique for challenging visceral vessel catheterization during complex aortic endovascular procedures, improving safety and ease of operation in demanding anatomies. By inflating a compliant balloon above the target arteries, it allows easy and safe advancement of the introducer sheath inside selected vessel, reducing shear forces and enhancing device pushability. This approach presents a useful, easy, and reproducible endovascular option that can be adopted for challenging vessel catheterization during advanced endovascular procedures.

Summary: This paper aims to assess the incidence and risk factors of bridging stent graft (BSG) fracture after thoracoabdominal aortic aneurysms branched endovascular aortic repair (BEVAR). The study found that BSG fracture is a rare but severe complication, not related to the type of stent used as bridging stent, but influenced by the use of multiple and different stents on the same vessel and the use of bare-metal stents for distal relining. Strict computed tomography angiography follow-up is recommended to detect target vessel instability.

Summary: For patients with extensive TAAAs treated with fenestrated/branched endovascular aortic repair, both historical and planned procedural staging were associated with reduced permanent SCI rates. However, no additional benefit was observed when procedural staging was performed in patients with historical staging and an intact collateral network. Further investigation is needed to explore the protective role of preoperative cerebrospinal fluid drainage placement.

Summary: This multicentric registry study compares the outcomes of using the anterior and posterior divisional branches of the internal iliac artery as distal landing zone for iliac branch devices (IBDs). The study found that both approaches had similar rates of technical success and adverse events. Summary: This study conducted a systematic review and meta-analysis of 47 studies and found that elderly patients with symptomatic carotid stenosis have a higher perioperative risk following carotid endarterectomy. The risk of stroke and death after surgery is significantly higher in octogenarians compared to nonoctogenarians. However, the perioperative stroke risk has declined in octogenarians over the past three decades. These findings suggest that selective urgent intervention should be considered for symptomatic elderly patients when medical therapy alone is ineffective.

Summary: This article presents the results of an unconventional endovascular treatment for a large (65 mm) saccular visceral aortic aneurysm in a 78-year-old patient who was deemed unfit for open surgery. The treatment involved the deployment of an aortic stent and embolization of the aneurysm sac using coils. The study concludes that this technique can be considered as an alternative when traditional treatments are not feasible. Rating: 8/10.
