Summary: In this retrospective study, reintervention after TEVAR was observed in 11.3% of patients with uncomplicated TBAD, and the main reasons included endoleak, aneurysmal dilation, and dissection progression. Larger initial aortic diameter and excessive proximal landing zone oversizing were identified as significant risk factors for reintervention. Reintervention did not significantly affect long-term survival rates.

Summary: The 5-year clinical outcomes of thoracic endovascular aortic repair for chronic type B aortic dissection were analyzed in this study. It was found that patients with adverse aortic events (AAEs) had higher rates of residual type A aortic dissection and aortic diameter >= 5.5 cm, as well as lower rate of complete false lumen thrombosis, and a longer interval from symptom onset to intervention. Partial or no false lumen thrombosis and aortic diameter >= 5.5 cm were identified as independent risk factors for AAEs.

Summary: This study describes two different situations of type A aortic dissection (TAAD) following thoracic endovascular aortic repair (TEVAR). The surgical methods used to repair TAAD after TEVAR are introduced, and the long-term prognosis is evaluated. The TAR and FET technique is found to be feasible and effective for treating complicated TAAD after TEVAR, with satisfactory surgical success rate and long-term prognosis.

This finding suggests clinical equipoise for the treatment of acute uncomplicated TBAD and supports the need for a larger randomized trial to determine whether early TEVAR provides clear benefits.

Summary: This study retrospectively evaluated the safety and efficacy of thoracic endovascular aortic repair (TEVAR) in patients with type B aortic dissection and identified risk factors for long-term mortality. The findings showed that TEVAR was associated with lower in-hospital mortality, death within 30 days, and aortic-related mortality compared to optimal medical treatment. Cox analysis also revealed that a vertical length of the dissection exceeding 150 mm increased the risk of mortality.

The results showed that over the past 7 years, the case volume of surgical residents steadily increased. Surgical trainees who graduated during the coronavirus pandemic have equal or greater total operative experience compared to trainees who graduated prior to the pandemic.

Summary: This study retrospectively analyzed the operative volume of transplant surgery residents from 1999 to 2021 using the ACGME database. The findings suggest that the number of transplant cases and resident involvement in transplant surgeries have remained consistent over the past 20 years, indicating that changes in operative volume are unlikely to be the cause of declining interest in transplant surgery among US trainees.

Summary: This study retrospectively analyzed the clinical data of HCC patients in a safety-net hospital and found that sociodemographic factors had no significant impact on the intervention and treatment of HCC, but insurance status was associated with the timing of HCC diagnosis. The results of this study indicate that safety-net hospitals can provide equitable treatment outcomes in managing HCC.

Summary: Over the past decade, neoadjuvant therapy has become the standard of care for borderline resectable and locally advanced pancreatic cancer. However, there is still debate regarding its value for clearly resectable disease. Meta-analyses suggest that neoadjuvant therapy can be offered as an acceptable standard of care, with recent studies showing superior survival for patients who tolerate neoadjuvant FOLFIRINOX. Ongoing randomized controlled trials are expected to provide more conclusive recommendations.
