Summary: A Doppler ultrasound of the scrotum is commonly used to improve the diagnostic confidence for testicular torsion in an emergency setting. However, the sensitivity of this investigation to identify torsion varies largely. This is due, in part, to a lack of guidelines on how-to perform the US and therefore training is necessary.

Summary: The study found that immediate contralateral orchiopexy (ICLO) at the time of scrotal exploration for testicular torsion suspicion was associated with a higher postoperative complication rate, especially in terms of hematoma and delayed wound healing. Therefore, ICLO should not be performed systematically.

Summary: The study aimed to determine the cut-off time to avoid orchiectomy in torsion of the spermatic cord. The results showed that the duration of pain can predict the probability of salvaging the testicles and performing orchiectomy.

Summary: Hemolymphangioma is a rare benign tumor that commonly occurs in the head and neck, but primary spermatic cord hemolymphangioma is extremely rare. Diagnosis can be challenging due to its rarity. A 15-year-old boy presented with scrotal complaints, and ultrasound revealed a cystic mass in the left spermatic cord region. He underwent surgery and was diagnosed with spermatic cord hemolymphangioma. After 1-month follow-up, there was no recurrence. This case highlights the importance of raising awareness for the diagnosis of spermatic cord hemolymphangioma in clinical practice.

Summary: The retrospective study analyzed the role, performance, and safety of Doppler ultrasonography before surgery for patients with suspected testicular torsion, demonstrating that DUS is safe, feasible, and useful in selected cases of suspected TT, but surgery should not be delayed in cases with a strong clinical suspicion.

Summary: Ultrasonography is an effective diagnostic tool for testicular torsion (TT), but there are reported cases of TT with preserved blood flow. We report a case of TT with preserved intratesticular flow on color Doppler ultrasound.

Summary: This study aimed to develop a decision tree model using US features to differentiate testicular torsion and other conditions of acute scrotum, and investigate predictive parameters of unsalvageable testis in testicular torsion. The results showed that the whirlpool sign of the spermatic cord, avascular nodule, and altered testicular vascularity were significant discriminators. Heterogeneous echotexture and symptom-to-operation delay were important prognostic factors for unsalvageable testis.

Summary: This study examines and compares the testicular salvage surgery rates for pediatric testicular torsion before and after the implementation of national quality metrics. The results show that the salvage surgery rate increased by 12.5% since the implementation of quality metrics. Patient factors such as age, race, distance to hospital, household income, insurance status, and use of ultrasound were similar between the two groups.

Summary: This study evaluated the safety of ultrasound in diagnosing pediatric acute scrotum and found that pre-hospital delay was the main factor affecting ischemic time in patients with testicular torsion. During the COVID-19 pandemic, there were no significant differences in time to treatment and outcome compared to pre-pandemic measures.

Summary: The study aimed to clarify the specific anatomical details of bell-clapper deformity (BCD) and found that BCD is best defined by complete investment of the testis, epididymis, and a length of the spermatic cord by the tunica vaginalis. The research revealed that the rate of BCD in scrotal testis ranged from 4.9% to 16%, with bilaterality in 66% to 100%, and bilaterality in cases of acute IVT ranged from 54% to 100%.

Summary: Recurrent Testicular Torsion (RTT) is a rarely reported condition after previous testicular torsion (TT) repair, and it shares similar signs and symptoms. This review examined the presentation, diagnosis, risk factors, management, and outcomes associated with RTT.

Summary: This study investigates the long-term effects of surgery for testicular torsion (TT) on endocrine and exocrine testicular function, oxidative stress in semen, and erectile function. The results show that TT does not significantly affect these functions in the long term. Preservation of the testis is associated with a higher round cell count and follicle stimulating hormone (FSH) compared to orchiectomy.

Summary: This study found that hospital transfer does not affect orchiectomy rates in pediatric patients with testicular torsion across all presentation timeframes. However, for patients presenting with testicular torsion early on (<24 hours of symptom onset), the delay associated with transfer has a detrimental effect on testicular viability.
