Summary: This study assessed the role of ventilation imaging in COVID-19 patients undergoing lung scintigraphy for suspected pulmonary embolism (PE). The findings showed that in this population, only 57% of patients could confidently exclude PE without ventilation imaging, while 31% of patients required ventilation imaging to rule out PE. The study also revealed a low prevalence of PE (12%).

Summary: This study assessed interobserver agreement for quantification of thromboembolic lesions in chronic thromboembolic pulmonary hypertension (CTEPH) using a CTPA scoring index. The results showed moderate agreement among different observers for the level of disease, with substantial agreement at the main/lobar level and weaker agreement at the segmental and subsegmental levels.Occlusions were the most reliable sign of chronic thromboembolism, and CT level of disease demonstrated good agreement between radiologists and was highly predictive of the surgical level in CTEPH.

Summary: This study aimed to differentiate between acute pulmonary embolism (PE) and chronic thromboembolic pulmonary hypertension (CTEPH) using dual-layer dual-energy computed tomography (dlDECT)-derived pulmonary perfusion maps. The study found that patients with acute PE had higher iodine uptake in normal perfused lung areas compared to those with CTEPH. Additionally, after normalizing iodine density in perfusion defects, patients with acute PE had reduced average perfusion compared to both CTEPH patients and controls. dlDECT enables quantification and characterization of pulmonary perfusion patterns in acute PE and CTEPH and can enhance differentiation between the two diseases when combined with morphological CT parameters.

The results showed that persistent dyspnea was associated with a lower QoL on the long term, however no relation was found between persistent perfusion defects and a lower QoL.

Summary: This study aimed to determine the prevalence of residual perfusion defects (RPD) in a cohort diagnosed with pulmonary embolism (PE) and the variables associated with RPD. The results showed that RPD were common in this group, and were associated with reduced pulmonary artery acceleration time, greater tricuspid annular plane systolic excursion (TAPSE), greater thrombotic burden, and unprovoked PE.

Summary: In a cohort of 27 consecutive patients with severe COVID-19 pneumonia, dual-energy CT demonstrated that areas of ground-glass opacifications and consolidations in the lungs showed a wide range of iodine uptake, indicating hypo- and hyperperfusion compared to normal lung tissue. Vessel enlargement within opacifications correlated significantly with iodine uptake levels. These findings suggest that DECT may help tailor therapy to individual patients based on predominant pathophysiology.

Summary: We report a case of a patient with chronic immune thrombocytopenia (ITP) who developed catastrophic antiphospholipid antibody syndrome (CAPS) following treatment with avatrombopag. The patient presented with symptoms including headache, nausea, and abdominal pain 3 weeks after initiation of avatrombopag, and diagnostic work-up revealed multiple microvascular thrombotic events. Laboratory tests showed positive antiphospholipid antibodies. The diagnosis of avatrombopag-associated CAPS was made.

Summary: Staphylococcus aureus infection is associated with increased levels of neutrophil extracellular traps (NETs) and von Willebrand factor (VWF), and with reduced activity of ADAMTS13. Peptidylarginine deiminase 4 (PAD4) contributes to NET formation and inactivates ADAMTS13 in vitro. The levels of PADs correlate with ADAMTS13 activity, with neutrophils as the likely source of PAD activity in S. aureus infection. NETs are positively correlated with PADs and negatively correlated with ADAMTS13 activity.

Summary: This study compared the safety and effectiveness of percutaneous direct puncture approach and contralateral femoral native vessel approach for catheter-directed thrombolysis of occluded infra-inguinal bypass grafts. The results showed that the direct puncture approach is a safe and efficient method with comparable clinical outcomes to the native vessel approach. The rate of adverse events was slightly lower in the direct approach group, but without significance.
