Haemostatic alterations in patients with cirrhosis and hepatocellular carcinoma: laboratory evidence and clinical implications

Summary: Venous thrombosis is a common complication in cancer, Hepatocellular carcinoma (HCC) is the most common primary liver cancer. It is associated with preexisting cirrhosis in 90% of cases. Patients with cirrhosis are more prone to bleed or thrombotic complications. There is evidence that HCC may increase the risk of venous thrombosis in cirrhosis patients. However, there are currently no specific guidelines for risk stratification and management of thromboprophylaxis in patients with cirrhosis and HCC.

Mechanisms of thrombosis in pancreatic ductal adenocarcinoma

Summary: This review summarizes the epidemiology and pathogenesis of thrombotic complications in patients with pancreatic cancer, and introduces novel mechanisms of hypercoagulability such as neutrophil extracellular traps and extracellular vesicles. The article also discusses the important role of these mechanisms in platelet and coagulation activation.

Assessing Clinically Meaningful Hypercoagulability after COVID-19 Vaccination: A Longitudinal Study

Summary: This study longitudinally evaluated coagulation profiles in healthy subjects who received COVID-19 vaccination and found that although there was a slight increase in thrombin generation in some subjects after vaccination, there was no clinically significant hypercoagulability. Platelet count, aggregation, and thromboelastometry parameters did not show relevant differences. No thrombotic or hemorrhagic events occurred.

Efficacy and safety of direct oral anticoagulants in older adults with atrial fibrillation: a prospective single-centre cohort study

Summary: Direct oral anticoagulants (DOACs) are underused in the elderly population, despite the evidence supporting their use. In this study, anticoagulant-naive patients aged 75 years and above were prospectively enrolled and stratified into older adults (aged 75-84 years) and extremely older adults (85 years and above). The results showed that extremely older adults had a higher incidence of thrombotic and hemorrhagic events compared to older adults in the first year of DOAC therapy. Several factors were associated with an increased risk of these events in both age groups.

Summary: This case-control study aimed to identify the effect of persistently increased plasma FXI levels on the risk of deep vein thrombosis (DVT). The study found that elevated FXI levels constitute a risk factor for DVT, and this risk nearly doubled in patients with persistently increased FXI levels.

Summary: This study assessed the role of platelets in thrombus and fibrin formation in patients with protein C or protein S deficiency under high-shear flow conditions. The results showed that these patients had impaired platelet activation, thrombus phenotype, and fibrin formation, but unchanged platelet adhesion. The defect may be caused by negative platelet priming.
