The decrease in procarboxypeptidase U (TAFI) concentration in acute ischemic stroke correlates with stroke severity, evolution and outcome

Background and objectives: Procarboxypeptidase U (proCPU, TAFI) concentration in plasma is potentially related to thrombotic tendency, and elevated proCPU levels have been reported in ischemic stroke patients. Improved insight into the role of proCPU in acute ischemic stroke is essential for the development of more adequate therapeutics that may include carboxypeptidase inhibitors. In this study we investigated whether the plasma concentration of proCPU and the proCPU kinetic profile in acute ischemic stroke are related to initial stroke severity, stroke evolution in the subacute phase and long-term stroke outcome. Methods: Plasma concentration of proCPU was assessed in 136 stroke patients at admission (7.5 h after stroke onset), at 24 h, at 72 h and at day 7 after stroke onset. We evaluated the relation between change in proCPU concentrations and (a) stroke severity (patients with TIA vs. stroke patients, NIHSS score at admission), (b) stroke evolution (stroke progression, infarct volume at 72 h), and (c) stroke outcome (mRS score at month 3). Results: ProCPU concentration decreased significantly in the first 72 h after stroke onset and thereafter returned to baseline. This biphasic time course, with its nadir at 72 h, was more pronounced in patients with severe stroke, unfavourable stroke evolution in the first 72 h and poor long-term outcome. Conclusions: The decrease in proCPU concentration in the first 72 h after stroke onset correlates with more severe stroke, unfavourable stroke evolution, and poor long-term stroke outcome.

Summary: The study aimed to evaluate the reliability of the wearable device Polar V800 in measuring RR intervals in FBSS patients, and found that the wearable device and 2-lead ECG have similar capabilities in capturing treatment responses. This suggests that a wearable heart rate monitor might be a reliable tool for detecting changes in chronic pain patients undergoing spinal cord stimulation therapy.

Summary: This study aimed to evaluate the effectiveness of HD-SCS after conversion from standard SCS and develop a model for predicting the long-term response of HD-SCS after unsatisfactory standard SCS. Results showed significant time effects for both low back and leg pain responders after conversion to HD-SCS. Logistic regression models highlighted the importance of pain intensity scores, medication use, paresthesia coverage (for back pain), and EQ5D (for leg pain) as predictors of being a responder after 12 months of HD-SCS.

Summary: This study found moderate to strong concordance between peripheral and central concentrations of kynurenine and 3-hydroxykynurenine, not only in psychiatric disorders, but in other physiological conditions. Despite discordant findings for other metabolites, blood metabolite levels were associated with clinical symptoms and treatment response in psychiatric patients, as well as with observed neuroanatomical abnormalities and glial activity.

Summary: The tryptophan catabolite (TRYCAT) pathway is involved in the pathophysiology of schizophrenia through the activation of the enzyme indoleamine-dioxygenase (IDO) by inflammatory and oxidative stress mediators. This systematic review examined the association between TRYCATs in the central nervous system (CNS) and peripheral blood in schizophrenia patients. The findings showed significant differences in TRYCAT levels between the CNS and peripheral blood, with only a modest increase in serum IDO activity associated with CNS findings.
