Summary: This study aimed to analyze the outcomes of surgical treatment for patients with vascular invasion of Masaoka stage III thymic epithelial tumors. The results showed satisfactory clinical outcomes, supporting the confidence in dealing with these complex surgeries effectively. Complete resection should be pursued, even if extensive vascular reconstructions are needed to achieve it.

Summary: This study retrospectively analyzed the postthymectomy outcomes and factors affecting the prognosis of thymomatous generalized myasthenia gravis (TGMG) in 86 patients. The results showed that male sex and disease duration < 11.5 weeks were independent predictors of complete stable remission (CSR) in TGMG postthymectomy. Patients with onset age < 52.8 years and symptoms of ocular and limb muscle weakness had a higher probability of achieving CSR compared to those with onset age > 52.8 years and symptoms of bulbar muscle weakness. Female sex was an independent predictor of MG symptom exacerbation in TGMG postthymectomy.

Preoperative seropositive acetylcholine receptor antibody, open thymectomy, non-R0 resection, WHO type B, and postoperative inflammation were identified as risk factors for PMG.

Summary: This study compared the use and 30-day postoperative outcomes of different surgical approaches for thymectomy. The results showed that VATS thymectomy was associated with decreased pulmonary complications and shorter hospital stay compared to the transsternal technique. No significant difference in outcomes was found between VATS and transcervical thymectomy. VATS had noninferior 5-year overall survival compared to the transsternal approach in thymic cancer patients. The study also indicated that the 30-day outcomes after thymectomy improved over time, which may be attributed to the increasing use of less invasive approaches.

Summary: Subxiphoid and subcostal thoracoscopic thymectomy appears to be a safe and feasible approach for treating MG and anterior mediastinal tumors. Out of 95 patients who underwent this approach, 93 achieved complete thymectomy and extended thymectomy. The mean operative time was 109 minutes with a mean estimated blood loss of 47 ml, and postoperative complications were generally low.

Summary: This study compared the outcomes of minimally invasive thymectomy (MIT) and open thymectomy (OT) in early or locally advanced thymoma patients. The results showed that MIT was associated with shorter operative times, shorter durations of hospital stay, and fewer complications compared to OT. However, there were no significant differences in long-term survival, recurrence-free survival, and complete resection between the two groups.

Summary: This retrospective study investigated patients undergoing surgery for thymoma or thymic carcinoma. The results showed that open surgery was associated with a higher incidence of complications, longer postoperative stay, and larger tumor size, while minimally invasive surgery was not. Extended resections were related to a higher incidence of complications, but VATS and RATS still ensured a lower incidence of complications and shorter postoperative stay in patients requiring extended resections. Symptomatic myasthenia patients might have a higher risk of more severe complications.

Summary: This study investigated the influence of tumor location along the first rib on survival in patients with non-small cell lung carcinomas invading the thoracic inlet. The results showed that tumor location did not significantly affect the rate of R0 resection and pathological complete response to induction therapy. However, posterior tumors, complete response to therapy, posterior location, and age less than 60 years were associated with better survival. Anterior tumors had a higher risk of systemic recurrence and derived greater benefit from pathological complete response.

Summary: Malignant mesothelioma (MESO) has different subtypes and epithelial-mesenchymal transition (EMT) phenotypes. A panel of MESO EMT genes were found to be correlated with hypermethylation of epigenetic genes and loss of CDKN2A/B expression. The MESO EMT genes were associated with activation of TGF-beta, hedgehog, and IL-2-STAT5 signaling, and suppression of IFN-alpha and IFN-gamma response. Immune checkpoints were upregulated, while certain immune receptors were downregulated with the expression of MESO EMT genes.
