Summary: The study examined the impact of preoperative aortic regurgitation on cusp repair necessity, finding that the repair rate increased with the severity of AR. While preoperative AR and cusp repair did not affect long-term survival and aortic valve reoperation, severe preoperative AR and multiple cusp repairs increased the risk of recurrent moderate-to-severe AR.

Summary: This study aimed to assess the echocardiographic criteria associated with aortic regurgitation (AR) in children with perimembranous ventricular septal defects (pm-VSD) during follow-up. A retrospective analysis of the echocardiographic evaluation of 40 children with restrictive pm-VSD was performed, and 15 patients with AR were matched to 15 patients without AR using propensity score. The results showed significant differences in aortic annulus z-score, Valsalva sinus z-score, sinotubular junction z-score, valve prolapse, and commissure commitment between the two groups. Aortic root dilatation, aortic valve prolapse, and commissure commitment to a perimembranous VSD are associated with AR.

Summary: This study evaluated the effect of aortic cross-sectional area on pressure recovery and effective aortic valve orifice area in patients with ascending aortic aneurysm and mildly diseased aortic valves. Progression to greater than mild aortic insufficiency is rare, and reoperation is rarely needed in midterm follow-up.

Summary: The aortic valve is a three-dimensional structure consisting of leaflets suspended within the functional aortic annulus. Disease of one component can independently lead to aortic valve dysfunction. However, as these structures are interconnected, disease of one component can cause abnormalities of the other over time. Therefore, aortic valve dysfunction is often multifactorial. This article provides a detailed account of the anatomical relationships relevant to valve-sparing root procedures.

Summary: Long-term outcomes of David V valve-sparing aortic root replacement (VSARR) in patients with bicuspid aortic valve (BAV) were compared to those with tricuspid aortic valve (TAV). The survival and reoperation rates after VSARR were similar between BAV and TAV patients, but a higher incidence of reoperation was observed in BAV patients in the late years postoperatively.
