Summary: This study conducted a systematic review and meta-analysis to assess the association between new-onset postoperative atrial fibrillation (POAF) after cardiac surgery and perioperative and long-term outcomes. The results showed that POAF was associated with an increased occurrence of various cardiovascular adverse events in the short and long term. However, the causal relationship of this association remains to be established.

Differences in long-term survival hazard may be due to the underlying pathophysiological risk factors for POAF, which differ by surgical procedure.

Summary: The study found an association between obesity and atrial fibrillation recurrence, with an increased risk of recurrence as BMI increased. However, the risk of adverse events did not increase linearly with obesity, showing a nonlinear relationship. Compared to the lower BMI group, the highest BMI group had not only a higher rate of atrial fibrillation recurrence, but also a significantly increased risk of adverse events.

Summary: This meta-analysis showed that corticosteroid significantly decreased the risk of POAF following cardiac surgery. However, corticosteroid did not have significant effects on other outcomes such as the length of ICU stay, hospital stay, infections, mortality, duration of ventilation, gastrointestinal complications, high blood sugar, stroke, postoperative bleeding, and myocardial infarction. The efficacy of corticosteroid in preventing POAF warrants further investigation due to heterogeneity among the included studies.

Further research is necessary to elucidate the underlying mechanisms linking POAF and long-term mortality.

Post-operative AF refers to new-onset AF after cardiac surgery within 4 weeks in patients without a previous history of AF. It is associated with short-term mortality and morbidity, but its long-term significance is not clear. This article reviews existing evidence and challenges in managing post-operative AF in cardiac surgery patients, discussing specific challenges in different phases of care.
