External risk factors can also impact vascular function through changes in VSMC morphology. Cerebrovascular reactivity (CVR) is a promising technique for identifying changes in cerebral smooth muscle state.

Summary: With the increasing research and services in the Mediterranean region, there are growing opportunities for global collaboration. The virtual Alzheimer's Association International Conference provided a platform for researchers to share updates on the understanding, treatment, and care of Alzheimer's disease and other dementias in the Mediterranean region. The conference highlighted the local epidemiology of AD and dementia, identified populations for genetic studies, emphasized the development of biomarkers for early diagnosis, and discussed culturally specific interventions for dementia prevention and improving quality of life.

Summary: This study found no significant difference in VRFs between autopsy-confirmed DLB and AD, but AD patients had higher rates of microinfarcts and amyloid angiopathy. Cerebral amyloid angiopathy in DLB was specifically correlated with memory and language. CAA pathology played an important role in DLB, and there were differences in CAA pathology between groups with high and intermediate likelihood of DLB.

Summary: White matter hyperintensities (WMH) are vascular lesions frequently seen in older adults, but recent evidence suggests that non-vascular mechanisms, particularly in Alzheimer's disease (AD), may also contribute to WMH. This alternative hypothesis is supported by arguments from neuropathology, neuroimaging, fluid biomarkers, and genetics. The underlying mechanisms of AD-related WMH, such as neurodegeneration or neuroinflammation, are discussed, as well as the implications for AD diagnosis and management. Testing this hypothesis and addressing remaining challenges can improve personalized care for patients.

Summary: This study investigates the links between CCR5 deficiency, neuronal sensitivity to oxidative stress, and the development of dementia. The results show that CCR5-& UDelta;32 allele, when combined with ApoE & epsilon;4, increases the risk for dementia, particularly vascular dementia. Additionally, Ccr5 deficiency in mice neurons leads to increased vulnerability to oxidative stress-induced cell death, suggesting a possible mechanism contributing to dementia.

The results showed that gout patients had a lower risk of all-cause dementia, AD, and VD, but the quality of evidence was generally low, and more studies are needed to validate and explore the mechanisms of this association.

Summary: Recent evidence suggests an association between pro-inflammatory diets and cognitive function, but few studies have explored this using the dietary inflammatory index (DII) and its relationship with different subtypes of dementia. This study analyzed data from UK Biobank participants without dementia and found that higher DII was associated with a higher risk of all-cause dementia and Alzheimer's dementia. However, the association with vascular and frontotemporal dementia was not significant.

Diagnostic criteria, including cognitive testing, neuroimaging, and fluid biomarkers, can improve accuracy of diagnosis and potentially guide prognosis. Large-scale studies are needed to determine the practical usefulness of combining multiple markers for clinical use.

Summary: The effects of COVID-19 on older people, especially those with dementia, are disproportionately negative, impacting health outcomes, care decisions, and leading to potential long-term neurological damage. Different responses to the pandemic from healthcare and social care systems in various countries highlight the need for specific attention to the presentation of the virus in older individuals and those with dementia, as well as the challenges of isolation and complex healthcare responses to minimize negative impacts. The identification of potential new working methods during the pandemic could offer a positive legacy from the crisis.
