Summary: Adenocarcinoma of the endometrium is the most common malignancy of the female genital tract in the United States, with an estimated 65,950 new cases in 2022 and 12,550 deaths. It includes pure endometrioid cancer and carcinomas with high-risk endometrial histology. Stromal or mesenchymal sarcomas are rare subtypes accounting for approximately 3% of all uterine cancers. This article focuses on the diagnosis, staging, and management of pure endometrioid carcinoma from the NCCN Guidelines for Uterine Neoplasms.

Summary: This study compared the diagnostic performance and inter-reader agreement of the 2005 and 2019 versions of the CT-based Bosniak classification systems for risk stratification of cystic renal lesions. The results showed that there was no significant improvement in diagnostic performance and inter-reader agreement between the two versions. However, the inter-reader agreement decreased when the 2019 version was dichotomized according to management strategy.

Summary: A 52-year-old woman suspected of uterine sarcoma underwent F-18-FDG PET/MRI, which revealed a myometrial mass and an endometrial lesion, suggesting dual primary neoplasms. Based on the PET/MRI findings, the intraoperative procedure was modified to determine the necessity of pelvic lymphadenectomy. PET/MRI was more useful than PET/CT in diagnosing and differentiating between the two malignant neoplasms in the uterus, due to the high contrast resolution and precise fusion of MRI.

Summary: This review article focuses on the treatment paradigms for early-stage endometrial cancer, particularly the role of external beam radiation therapy. Surgical resection is the main treatment, and adjuvant radiation therapy after surgery aims to prevent local recurrence. Observation is a possible approach for low-risk patients, while vaginal cuff brachytherapy is recommended for high-intermediate risk patients. Pelvic radiation therapy is reserved for deeply invasive disease and non-endometrioid histologies. Definitive treatment for medically inoperable patients includes brachytherapy +/- pelvic external beam radiation therapy.

Summary: The American Radium Society has summarized evidence-based guidelines for the management of uterine clear cell and serous carcinomas, which have a high risk of locoregional and distant spread, worse prognosis than endometrioid cancers, and may benefit from multimodal adjuvant therapies. Complete surgical staging is recommended as the primary treatment, and counseling patients about combined-modality treatment with systemic chemotherapy and radiotherapy could be reasonable. Further research is needed to determine optimal sequencing of therapy and appropriate management based on individual risk factors.

Summary: This study analyzed the clinical and reproductive outcomes of patients with uterine smooth muscle tumor of uncertain malignant potential (STUMP) who underwent myomectomy. The results showed that uterus-saving procedures and fertility-preservation strategies in STUMP patients are feasible, safe, and associated with a low risk of malignant recurrence. Additionally, laparoscopic surgery can be considered as the preferred surgical approach.

Summary: The use of combined oral contraceptives and progestin-only contraceptives is associated with a decreased risk of ovarian cancer. Specifically, the injectable progestin-only contraceptive depot-medroxyprogesterone acetate (DMPA) was found to be associated with a 35% decreased risk of ovarian cancer, with a significant trend of decreasing risk with increasing duration of use. Some studies showed an inverse association between DMPA use and ovarian cancer risk, while others showed increased risk, indicating a need for further investigation into the mechanism of this association.
