Background Few authors have been attempting between mast cells and dermal dendrocytes interactions on urticaria. Objective To describe the extruded mast cell granules and dermal dendrocytes in drug-induced acute urticaria. Methods Seven patients with drug-induced acute urticaria were enrolled in the study. We token skin biopsies of urticaria lesion and perilesional skin. The 14 fragments collected were processed to immunogold electron microscopy using single stains to tryptase and FXIIIa, besides double immunogold labeling with both. Results Some sections demonstrated mast cells in degranulation process, both in anaphylactic and piecemeal degranulation types. After double immunogold staining, 10 nm (FXIIIa) and 15 nm (Tryptase) gold particles were present together over the granules in mast cells indicating that tryptase and FXIIIa are each localized within the granules of these cells. Interestingly, we found a strong evidence of than the exocytosed mast cell granules contents both FXIIIa and tryptase immunolabeled are phagocytized by dermal dendrocytes. Conclusions The current observations provide morphological evidence that the exocytosis-phagocytosis mechanisms of mast cell granules represents one pathophysiological example of mast cells-dermal dendrocytes interactions in urticaria.

Inflammasome and Inflammatory Programmed Cell Death in Chromoblastomycosis

Carla Pagliari, Luciane Kanashiro-Galo, Mirian Nacagami Sotto

Summary: This study investigated the presence and potential role of inflammasome elements in cutaneous lesions of Chromoblastomycosis. The results suggest that inflammasome-mediated response and pyroptosis may play a role in the immune response against fungal agents of CBM.

Eosinophilia and elevated IgE serum levels: a red flag: when your diagnosis is not a common atopic eczema or common allergy

Paulo Ricardo Criado, Helio Amante Miot, Mayra Ianhez

Summary: This study aims to explore the biological and clinical spectrum of cutaneous manifestations involving tissue and/or systemic eosinophilia and distinct serum levels of IgE. A diagnostic approach for patients with elevated serum IgE level is provided, including conditions not related to atopic dermatitis. Clinical patterns of skin diseases associated with tissue eosinophilia are also listed to aid in diagnosis.

Obesity Induces an Impaired Placental Antiviral Immune Response in Pregnant Women Infected with Zika Virus

Anna Claudia Calvielli Castelo Branco, Emily Araujo De Oliveira, Natalli Zanete Pereira, Ricardo Wesley Alberca, Amaro Nunes Duarte-Neto, Luiz Fernando Ferraz Da Silva, Fernanda Guedes Luiz, Naiura Vieira Pereira, Mirian Nacagami Sotto, Naiara Naiana Dejani, Patricia Helen Carvalho Rondo, Elyzabeth Avvad-Portari, Zilton Farias Meira De Vasconcelos, Alberto Jose da Silva Duarte, Tamiris Azamor, Maria Notomi Sato

Summary: The incidence of obesity is increasing worldwide, particularly in women, and this can affect pregnancy outcomes. During this period, viral infections pose a risk to the mother, placenta, and fetus. The Zika virus outbreak in Brazil has resulted in the congenital Zika syndrome (CZS) and devastating consequences such as microcephaly in newborns. This study examines the impact of maternal overweight/obesity on the expression of antiviral factors in the placental tissue of Zika-infected mothers.

JAK-STAT pathway inhibitors in dermatology

Helio Amante Miot, Paulo Ricardo Criado, Caio Cesar Silva de Castro, Mayra Ianhez, Carolina Talhari, Paulo Mueller Ramos

Summary: The JAK-STAT signaling pathway plays important roles in immune response, carcinogenesis, cell differentiation, division, and death. Drugs that interfere with different JAK-STAT signaling patterns have potential indications for various medical conditions. In dermatology, JAK-STAT pathway inhibitors have been primarily used for treating inflammatory or autoimmune diseases, but ongoing investigations are expanding their potential indications.

Summary: In this study, histopathological analysis of liver samples from 70 patients who died due to infections and complications of yellow fever, dengue fever, and chikungunya fever was conducted. The results showed significant differences in histopathological alterations between the control and infection groups, with a predominance of changes in the midzonal area in all three cases analyzed. Hepatic involvement in yellow fever cases showed more severe histopathological changes.

Carla Pagliari, Luciane Kanashiro-Galo, Mirian Nacagami Sotto

Summary: This study aimed to identify the mechanisms of programmed cell death and inflammasome components in human oral mucosal lesions of paracoccidioidomycosis. The results showed increased expression of IL-1β, NLRP-1, caspase-1, and RIPK-3, suggesting the involvement of inflammasome complex in the immunopathogenesis of oral lesions in paracoccidioidomycosis.

Chronic Urticaria and Angioedema: Masqueraders and Misdiagnoses

Jonathan A. Bernstein, Navid Ziaie, Roberta Criado, Paulo Ricardo Criado, Stephanie Rea, Mark Davis

Summary: Chronic urticaria is a common condition characterized by intensely itchy wheals that last for at least 6 weeks. It can be either spontaneous or inducible, with specific triggers in the latter case. Angioedema, which is a localized swelling involving deep layers of the skin, can occur separately or in conjunction with chronic urticaria. Both conditions have multiple mimics, and it is important to consider a broad range of differential diagnoses to avoid incorrect diagnosis and ensure appropriate investigation, treatment, and prognosis for the patient.
