ObjectivesTo provide a unique perspective on geriatric headache and a number of novel treatment options that are not well known outside of the headache literature. DesignReview of the most current and relevant headache literature for practitioners specializing in geriatric care. ResultsEvaluation and management of headache disorders in older adults requires an understanding of the underlying pathophysiology and how it relates to age-related physiological changes. To treat headache disorders in general, the appropriate diagnosis must first be established, and treatment of headaches in elderly adults poses unique challenges, including potential polypharmacy, medical comorbidities, and physiological changes associated with aging. ConclusionThe purpose of this review is to provide a guide to and perspective on the challenges inherent in treating headaches in older adults. J Am Geriatr Soc 66:2408-2416, 2018.

Race, Ethnicity, and Other Sociodemographic Characteristics of Patients with Hospital Admission for Migraine in the United States

Summary: This study investigated the impact of race and income on patients with migraine admitted to the hospital. The results showed that African American, Hispanic, and Native American patients were more likely to have lower household income, while Caucasian patients had higher income. However, race and income did not affect the length of hospital stay.

Pain sensitivity in relation to frequency of migraine and tension-type headache with or without coexistent neck pain: an exploratory secondary analysis of the population study

Summary: This study investigated whether coexistent self-reported neck pain influences pain sensitivity in individuals with migraine and tension-type headache. The results showed that individuals with chronic headache had higher pericranial tenderness than controls, and this difference was more significant in individuals with neck pain. Coexistent neck pain was associated with greater pericranial tenderness in individuals with chronic headache and to a lesser degree in those with episodic headache.

Prevalence and clinical features of hemicrania continua in clinic-based studies: A systematic review and meta-analysis

Summary: This study aims to estimate the relative frequencies and clinical features of hemicrania continua in adult patients with headache in a clinical setting. The meta-analysis of multiple studies found that the relative frequency of hemicrania continua was 1.8%, with the most common symptoms being lacrimation, conjunctival injection, and restlessness/agitation. The study highlights the need for standardized data acquisition and reporting to accurately determine the prevalence of hemicrania continua and increase awareness of its clinical importance.

Summary: The use of narrow band green light has been found to reduce anxiety symptoms in patients with generalized anxiety disorder (GAD). Therefore, it is suggested that incorporating narrow band green light into psychotherapy sessions may help alleviate anxiety in GAD patients.

Summary: This study used quantitative sensory testing to analyze the presence of cephalic and extracephalic allodynia in the pre-treatment non-ictal phase of migraine patients. The results showed that the presence of cephalic allodynia was highly correlated with the response to galcanezumab treatment, suggesting that it can be used to predict the treatment outcome.

Neck pain and headache: Pathophysiology, treatments and future directions

Summary: Neck pain is a common complaint in the general population, often associated with primary headache disorders such as migraine and tension-type headache. There is a high prevalence of neck pain in people with migraine or tension-type headache, and it has been identified as a risk factor for these headaches. The exact mechanisms linking neck pain to migraine and tension-type headache are uncertain, but pain sensitivity appears to play a role.

Migraine and subsequent head and neck cancer: A nationwide population-based cohort study

The article also discusses future research directions to address diagnostic challenges and improve the understanding and management of these conditions.
