The Impact of b/tsDMARD Dose Reduction on Chronic Hepatitis B in Rheumatoid Arthritis Patients: A Two-Center Long-Term Safety Analysis

Der-Yuan Chen, Hsin-Hua Chen, Shih-Hsin Chang, Yi-Ming Chen, Po-Hao Huang, Chia-Wei Hsieh, Joung-Liang Lan, Kuo-Tung Tang

Summary: This study investigated the change of hepatitis B virus (HBV) viral loads and HBV reactivation (HBVr) in rheumatoid arthritis (RA) patients after tapering the dose of biological/targeted synthetic disease-modifying antirheumatic drugs (b/tsDMARDs). The results showed that the HBV viral load decreased after the tapering of b/tsDMARDs in RA patients with detectable HBV DNA. Therefore, dose reduction in b/tsDMARDs may be beneficial for RA patients.

Serious Infection Rates Among Patients with Select Autoimmune Conditions: A Claims-Based Retrospective Cohort Study from Taiwan and the USA

Wen-Nan Huang, Ching-Yi Chuo, Ching-Heng Lin, Yi-Ming Chen, Wei-Szu Lin, Katie Tuckwell, Nicholas S. S. Jones, Joshua Galanter, Lisa Lindsay

Summary: This study aimed to estimate the serious infection rates among patients with select autoimmune conditions in Taiwan and the USA. The results showed that patients with autoimmune conditions had significantly higher serious infection rates compared to the general population in both settings, driven mainly by bacterial, respiratory, urinary tract, and opportunistic infections. Patients with lupus nephritis had the highest burden of serious infections relative to the general population in Taiwan and the USA.

Sex-specific impacts of social isolation on loneliness, depressive symptoms, cognitive impairment, and biomarkers: Results from the social environment and biomarker of aging study

Fei-Yuan Hsiao, Li-Ning Peng, Wei-Ju Lee, Liang-Kung Chen

Summary: This study investigates the sex-specific associations between social isolation and psychological outcomes and biomarkers in a nationally representative population. The findings show that social isolation is associated with a higher risk of loneliness in women but not in men, and marriage is protective against loneliness for men. Social isolation is not significantly associated with depressive symptoms or cognitive impairment. The results highlight the importance of considering gender differences in the relationship between social isolation and psychological outcomes.

Prefrailty subtypes differentially predict 5-year mortality in the functionally independent geriatric population

Yu-Shan Lee, Yi-Ming Chen, Yin-Yi Chou, Chu-Sheng Lin, Jun-Peng Chen, Fu-Hsuan Kuo, Cheng-Fu Lin, Shih-Yi Lin

Summary: This study compared the 5-year mortality among functionally independent geriatric outpatients with different prefrailty subtypes and frailty. The results showed a high proportion of prefrailty and frailty, and the PF2 subtype and frailty were significantly associated with 5-year mortality risk.

Health care utilization and potentially preventable adverse outcomes of high-need, high-cost middle-aged and older adults: Needs for integrated care models with life-course approach

Lin-Chieh Meng, Shih-Tsung Huang, Ho-Min Chen, Ardeshir Z. Hashmi, Fei-Yuan Hsiao, Liang-Kung Chen

Summary: This study aimed to explore the health care utilization and risk of preventable adverse outcomes stratified by age and the high need, high cost (HNHC) state. HNHC participants had higher multimorbidity frailty index (mFI), drug consumption, and health care utilization. They also had a significantly higher risk of preventable hospitalizations, preventable emergency department visits, and mortality.

The role of ultrasound synovitis scores for patients who are at risk of rheumatoid arthritis

Tsai-Hung Yen, Yi-Da Wu, Hsin-Hua Chen, Chih-Wei Tseng, Yi-Hsin Chen, Yi-Ming Chen, Kuo-Lung Lai

Summary: This study found that patients at risk for rheumatoid arthritis (RA) had higher ultrasound synovitis scores compared to non-RA patients, and patients with RA had higher synovitis scores than those at risk for RA.

Clinical impacts of frailty on 123,172 people with diabetes mellitus considering the age of onset and drugs of choice: a nationwide population-based 10-year trajectory analysis

Shih-Tsung Huang, Liang-Kung Chen, Fei-Yuan Hsiao

Summary: This study aimed to investigate the interrelationships between the age of onset, frailty, anti-diabetic medications and clinical outcomes in people with diabetes mellitus (DM). The proportion of frailty increased with age at diagnosis amongst people with incident DM and the mFI scores increased significantly during the 10-year follow-up. Frailty assessment and management should be implemented into routine diabetes care.

The diagnostic and prognostic value of a line immunoblot assay in Taiwanese patients with systemic sclerosis

Tsai-Hung Yen, Jun-Peng Chen, Tsu-Yi Hsieh, Wei-Ting Hung, Kuo-Lung Lai, Chia-Wei Hsieh, Hsin-Hua Chen, Wen-Nan Huang, Yi-Hsing Chen, Yi-Ming Chen

Summary: This study aimed to assess the diagnostic performance and prognostic value of disease-specific antibodies and anti-Ro52 in Taiwanese SSc patients using a commercial line immunoblot assay (LIA). The study found that LIA exhibited a sensitivity and specificity of 65.4%. It was also observed that the presence of anti-Ro52 or the absence of SSc-specific autoantibodies was associated with more advanced diseases in SSc patients. The combination of IIF and LIA testing may improve the diagnostic specificity of SSc.

Potential alleviation of bone mineral density loss with Janus kinase inhibitors in rheumatoid arthritis

Yun-Wen Chen, Hsin-Hua Chen, Wen-Nan Huang, Jun-Peng Chen, Yi-Hsing Chen, Yi-Ming Chen

Summary: The study found that Janus kinase inhibitors (JAKi) may be more effective in preventing bone loss compared to other targeted therapies, especially in rheumatoid arthritis (RA) patients who are at risk of bone loss and test positive for anti-cyclic citrullinated peptide antibody (ACPA). JAKi could be a potential therapeutic option to mitigate generalized bone loss.

Using Hypothesis-Led Machine Learning and Hierarchical Cluster Analysis to Identify Disease Pathways Prior to Dementia: Longitudinal Cohort Study

Shih-Tsung Huang, Fei-Yuan Hsiao, Tsung-Hsien Tsai, Pei-Jung Chen, Li-Ning Peng, Liang-Kung Chen

Summary: This study used machine learning methods to identify 49 hierarchical disease triplet clusters related to dementia development, with cardiovascular and cerebrovascular diseases playing leading roles and mental conditions, locomotion difficulties, infections, and nonspecific neurological conditions playing supporting roles. This research is of great significance for the prevention and care of dementia.

Higher risk of future events, mortality and greater healthcare use among patients with increasingly recurrent atherosclerotic cardiovascular disease events in Taiwan: a retrospective cohort study

Chia-Yun Hsu, Wen-Jone Chen, Hung-Ju Lin, Ho-Min Chen, Yea-Harn Yang, Wei-Ju Chen, Chieh-Min Chen, Fei-Yuan Hsiao

Summary: The study aimed to describe the occurrence of recurrent atherosclerotic cardiovascular disease (ASCVD) events within 3 years after a new-onset event, the associated disease burden and statin prescribing in patients with ASCVD in Taiwan. The results showed that the rates of recurrent events increased gradually, and patients had higher mortality rates, hospital readmission rates and emergency room visit rates with each recurrent event. However, statin prescriptions were suboptimal at both the index event and after each recurrent event.
