Surgery of pancreas tumors in pediatric and adolescent patients: a single institution experience in South America

Summary: The study evaluated short- and long-term outcomes of pediatric and adolescent patients who underwent surgical resection of pancreatic tumors at the National Institute of Neoplastic Diseases INEN in Peru from 2000 to 2020. Results showed that solid pseudopapillary neoplasm (SPN) was the most common type of pancreatic tumor, with favorable survival rates post-surgery and acceptable postoperative complications.

Summary: This study found that preoperative estimation of hepatic functional reserve using the indocyanine green retention test can help predict portal pressure in cirrhotic patients, thereby predicting postoperative liver function outcomes. The ICG-R15 value has good accuracy in predicting portal hypertension.

Improved Survival over Time After Resection of Colorectal Liver Metastases and Clinical Impact of Multigene Alteration Testing in Patients with Metastatic Colorectal Cancer

Summary: Survival in patients undergoing resection of colorectal liver metastases has improved over the past 20 years, associated with the introduction of irinotecan- and/or oxaliplatin-containing regimens, molecular targeted therapy, and multigene alteration testing. Liver resectability indications expanded over time, with a significantly better 5-year overall survival rate in 2006-2014 compared to 2001-2005. RAS alteration was associated with worse survival outcomes, while multigene alteration testing proved useful for prognostication and identification of potential therapeutic targets.

Understanding conditional cumulative incidence of complications following liver resection to optimize hospital stay

Summary: This study assessed the changes in risks of complications after liver resection by stratifying surgical complexity. The cumulative incidence rate of treatment-requiring complications was found to be higher after high complexity liver resection compared to low complexity and intermediate complexity resections. Without cirrhosis/chronic hepatitis, the incidence rate of treatment-requiring complications decreased over time.

Conversion surgery after preoperative therapy for advanced hepatocellular carcinoma in the era of molecular targeted therapy and immune checkpoint inhibitors

Summary: This review article examines the optimal strategies for advanced hepatocellular carcinoma (HCC) tumors, including vascular tumor thrombus and extrahepatic metastases. The study finds that conversion surgery after molecular targeted therapy and immune checkpoint inhibitors is feasible and can improve outcomes for initially unresectable HCC. Combination therapy using lenvatinib, atezolizumab, and bevacizumab shows promise in increasing overall survival and tumor response rates. Further clinical investigation is needed to distinguish between technically unresectable and oncologically unresectable but technically resectable tumors.

Clinicopathological factors associated with recurrence in patients undergoing resection of pancreatic solid pseudopapillary neoplasm

Summary: Solid pseudopapillary neoplasm (SPN) is frequently diagnosed in young women and has a good overall prognosis after R0 surgical resection, even in cases of recurrence. Age ≥ 28 years, tumor diameter ≥ 10 cm, invasion of adjacent organs, lymph node metastasis (pN +), and AJCC Stage III were identified as predictors for recurrence in resected SPN.
