Summary: This large registry-based study is the first to document the incidence and outcomes of open conversion after EVAR in a national cohort. Women, patients with large aneurysms and complex anatomy, as well as those who undergo urgent procedures are at a higher risk of open conversion. Conversion is associated with significant morbidity and mortality, highlighting the importance of appropriate patient selection during EVAR.

Summary: This study aims to evaluate the mid-term outcomes of fenestrated endovascular aneurysm repair (FEVAR) for proximal aortic pathology after previous open surgical repair (OSR). The study found that FEVAR is a safe and effective alternative for the endovascular treatment of para-anastomotic aneurysms/pseudoaneurysms after OSR, with high technical success, low mortality and morbidity, and good mid-term outcomes.

Summary: This study aims to define the indications for open aneurysm repair (OAR) and its impact on outcomes in the era of endovascular aneurysm repair (EVAR). The results show that no single anatomical contraindication is a predictor of worse outcomes with OAR in patients who do not meet the criteria for EVAR. Patients who choose OAR based on preference have lower rates of perioperative complications, shorter hospital stays, and superior long-term survival rates compared to those who undergo OAR due to anatomical or systemic contraindications to EVAR.

Summary: The study aimed to quantify the impact of endovascular aneurysm repair (EVAR) on 1-year mortality in elderly patients with abdominal aortic aneurysms (AAAs). The results showed that EVAR significantly decreased 1-year mortality rates compared to non-operative management in elderly patients, but the survival benefit was mainly seen in patients with specific conditions such as large AAA diameters and Gagne Indices. Patient selection is crucial for realizing the benefits of EVAR in elderly individuals.

Summary: In this study comparing aortoduodenal fistulas (ADFs) after endovascular abdominal aortic aneurysm repair (EVAR) vs after open aortic repair (OAR), it was found that patients in the OAR group had significantly higher postoperative complication rates compared to the EVAR group. The most common bacteria identified in blood cultures were Escherichia coli, with Candida spp. being the predominant pathogen found on intra-abdominal smears.

Summary: This study evaluated the outcomes of endovascular aortic repair (EVAR) in patients with abdominal aortic aneurysms who were deemed unfit for open repair. The results showed that EVAR improved actual 1-year mortality rates compared to predicted mortality rates, especially in UNFIT patients with smaller Gagne Indices and larger aneurysm diameters. However, patients deemed unfit for open repair due to frailty or multiple reasons had higher mortality rates and worse 1-year survival.

Summary: This study reviewed open abdominal aortic surgery cases performed at a tertiary care center from 2007 to 2020 and found that complex open aortic aneurysm repair (OAR) comprised the majority, but were not accounted for in the Vascular Quality Initiative (VQI). The study suggests the creation of a VQI module for complex OAR to improve benchmarking for volume and outcomes in aortic surgery.

Summary: The diameter of aortic abdominal aneurysm (AAA) is associated with late outcomes following repair, with patients with large AAA having higher rates of reintervention, rupture, mortality, and loss to follow-up after EVAR compared to those with smaller AAA or after open repair. Patients with large AAAs who are medically fit should consider undergoing open repair for better long-term outcomes and rigorous follow-up after EVAR is essential.

Summary: Patients undergoing pancreaticoduodenectomy (PD) at low or mixed-volume hospitals have higher risks of short-term complications and 90-day mortality compared to those treated at high-volume PD centers. However, for older and medically complex patients with pancreatic adenocarcinoma, the operative experience of the center is not as significant as undergoing surgery at a high-volume PD center.

Summary: The objective of this study was to assess the operative experience and overall case volume of surgery residents before and during the pandemic. The results showed that over the past 7 years, the case volume of surgical residents steadily increased. Surgical trainees who graduated during the coronavirus pandemic have equal or greater total operative experience compared to trainees who graduated prior to the pandemic.

Summary: This study retrospectively analyzed the operative volume of transplant surgery residents from 1999 to 2021 using the ACGME database. The findings suggest that the number of transplant cases and resident involvement in transplant surgeries have remained consistent over the past 20 years, indicating that changes in operative volume are unlikely to be the cause of declining interest in transplant surgery among US trainees.

Summary: This study retrospectively analyzed the clinical data of HCC patients in a safety-net hospital and found that sociodemographic factors had no significant impact on the intervention and treatment of HCC, but insurance status was associated with the timing of HCC diagnosis. Summary: Over the past decade, neoadjuvant therapy has become the standard of care for borderline resectable and locally advanced pancreatic cancer. However, there is still debate regarding its value for clearly resectable disease. Meta-analyses suggest that neoadjuvant therapy can be offered as an acceptable standard of care, with recent studies showing superior survival for patients who tolerate neoadjuvant FOLFIRINOX. Ongoing randomized controlled trials are expected to provide more conclusive recommendations.
