Summary: This retrospective cohort study compared the long-term outcomes of endovascular and surgical revascularization in PAD patients and found that although endovascular revascularization can reduce the risk of MALE, it is associated with higher mortality. Surgical revascularization has lower long-term mortality, which contradicts previous studies and highlights the controversy in this field.

Summary: Despite high female prevalence of PAD, little is known about outcomes after LER for symptomatic PAD. In the VOYAGER PAD study, low-dose rivaroxaban reduced cardiovascular and limb events for patients with PAD undergoing LER. Women may have higher risk of unplanned index limb revascularization and were more likely to discontinue treatment.

Summary: This study aimed to describe sex-based differences in clinical characteristics and outcomes among hospitalized patients affected by peripheral arterial disease (PAD). The study found that although there were some differences between sexes among PAD patients, gender did not significantly affect the occurrence of major adverse cardiovascular events (MACEs) and major adverse limb events (MALEs). However, among these patients, males were more likely to experience adverse clinical outcomes.

Summary: The study found that the use of paclitaxel drug-coated devices in patients undergoing peripheral artery disease lower extremity revascularization was not associated with mortality or major adverse limb events, but was associated with a persistent reduction in unplanned index limb revascularization.

Summary: Recurrent limb events after endovascular revascularization of the superficial femoral artery (SFA) do not increase the risk of all-cause or cause-specific mortality, and should not discourage repeat revascularization to relieve symptoms or ischemia.

Summary: The study aimed to quantify the regional variation in revascularization and amputation rates for peripheral artery disease (PAD). A repeated cross-sectional analysis of Ontarians aged 40 years and older between 2002 and 2019 was conducted. The results showed significant regional differences in demographics, comorbidities, and rates of revascularization and amputation for PAD.

Summary: This study found that revascularization procedures have significant effects on mitochondrial function and content in ischemic muscles, with no significant changes in non-ischemic muscles. This indicates that revascularization can promote mitochondrial recovery in affected muscle regions.

Summary: Routine coronary angiography (CAG) in peripheral arterial disease (PAD) patients undergoing percutaneous transluminal angioplasty (PTA) may not provide a significant advantage in predicting long-term survival compared to PAD patients without CAD. While PAD patients with CAD showed no significant difference in mortality compared to those without CAD, they were more likely to undergo repeat percutaneous coronary intervention (PCI). Routine CAG and subsequent PCI, if needed, may be a reasonable strategy for reducing mortality risk in PAD patients.

Summary: This article summarizes recent research on treatment targets for peripheral artery disease (PAD)-related limb ischemia. The current treatments for PAD have limitations and complications, and new therapies including exercise programs, drugs, stem cell treatments, and RNA therapeutics are being developed and tested. Multiple clinical trials are expected to provide results within the next 5 years.

Summary: Peripheral artery disease (PAD) is a common condition with poor prognosis, and paclitaxel-coated devices (PCDs) have shown progress in treatment, but recent studies have raised controversies. The FDA recommends restricting the use of PCDs and emphasizes the critical role of real-world data in evaluating PCDs.

Summary: Limited data suggests that the use of ultrasound enhancing agent (UEA) is associated with changes in clinical management and lower mortality in intensive care unit (ICU) patients. A retrospective observational study was conducted to determine if contrast echocardiography (vs non-contrast echocardiography) is associated with differences in length of stay (LOS) and subsequent resource utilization in ICU setting. The results showed that Definity-enhanced echocardiography in the ICU setting is associated with lower rates of subsequent TTE and TEE during the index hospitalization, and shorter ICU LOS.

Summary: This study found that a concordant left ventricular (LV) lead placement in cardiac resynchronization therapy (CRT) was associated with better outcomes, including reduced death and heart failure hospitalization, decreased LV end systolic volume, and increased ejection fraction. Further research is needed to explore feasible methods for achieving concordance in LV lead placement.
