An algorithmic approach to balloon undilatable coronary lesions

Athanasios Rempakos, Spyridon Kostantinis, Bahadir Simsek, Judit Karacsonyi, Salman Allana, Mohaned Egred, Hani Jneid, Kambis Mashayekhi, Carlo Di Mario, Oleg Krestyaninov, Dmitrii Khelimski, Anastasios Milkas, Yader Sandoval, M. Nicholas Burke, Emmanouil S. S. Brilakis

Summary: Balloon undilatable lesions are lesions that cannot be expanded despite successful cross by a guidewire and a balloon. These lesions can be either de novo or in-stent. A systematic approach using various techniques such as high-pressure balloon inflation, plaque modification balloons, intravascular lithotripsy, very high-pressure balloon inflation, coronary atherectomy, laser coronary angioplasty, and extraplaque lesion crossing can effectively treat these lesions, increasing procedure efficiency, success, and safety.

Prevalence of anxiety and depression symptoms in a sample of outpatients with ATTR cardiac amyloidosis

Martina Smorti, Lucia Ponti, Francesco Soffio, Alessia Argiro, Federico Perfetto, Mattia Zampieri, Carlotta Mazzoni, Alessia Tomberli, Marco Allinovi, Carlo Di Mario, Iacopo Olivotto, Francesco Cappelli

Summary: Patients with ATTR cardiac amyloidosis (ATTR-CA) often experience high levels of anxiety and depression. Older patients, especially females, with more advanced disease are at a higher risk of developing an anxious or depressive disorder. It is important for physicians to take into consideration the psychological well-being of ATTR-CA patients and provide them with appropriate support to reduce disease burden and improve quality of life.

Management strategies for heavily calcified coronary stenoses: an EAPCI clinical consensus statement in collaboration with the EURO4C-PCR group

Emanuele Barbato, Emanuele Gallinoro, Mohamed Abdel-Wahab, Daniele Andreini, Didier Carrie, Carlo Di Mario, Dariusz Dudek, Javier Escaned, Jean Fajadet, Giulio Guagliumi, Jonathan Hill, Margaret McEntegart, Kambis Mashayekhi, Nikolasos Mezilis, Yoshinobu Onuma, Krzyszstof Reczuch, Richard Shlofmitz, Giulio Stefanini, Giuseppe Tarantini, Gabor G. Toth, Beatriz Vaquerizo, William Wijns, Flavio L. Ribichini

Summary: Since the publication of the 2015 EAPCI consensus, there has been a significant increase in PCI performed in cases of severely calcified coronary artery disease. This has been driven by the growing need for interventions in elderly patients and the availability of new technologies. The current EAPCI clinical consensus statement provides comprehensive guidance on the management of heavily calcified coronary stenoses, including imaging assessment, tool and technique selection, and considerations for complications and training.

Margarita Brida, Salvatore De Rosa, Antoine Legendre, Magalie Ladouceur, Laura Dos Subira, Giancarlo Scognamiglio, Carlo Di Mario, Jolien Roos-Hesselink, Eva Goossens, Gerhard Diller, Michael A. Gatzoulis

Summary: The rates of successful surgical repair and life expectancy for patients with congenital heart disease have significantly increased in recent decades. However, exposure to cardiovascular risk factors has led to a shift in the clinical trajectory of adult congenital heart disease, with an increasing burden of acquired cardiovascular disease, primarily atherosclerotic cardiovascular disease (ASCVD). Currently, comprehensive guidance for the prevention and management of acquired heart disease in adult congenital heart disease patients is lacking. This document provides an overview of acquired heart disease and offers guidance on ASCVD prevention for both specialists and non-specialists in adult congenital heart disease, aiming to reduce the cardiovascular burden in this growing population.

Is Height2.7 Appropriate for Indexation of Left Ventricular Mass in Healthy Adolescents? The Importance of Sex Differences

Hannah C. M. Taylor, Nishi Chaturvedi, George Davey Smith, Diana L. S. Ferreira, Abigail Fraser, Laura D. Howe, Alun D. Hughes, Debbie A. Lawlor, Nic J. Timpson, Chloe M. Park

Summary: This study investigated the appropriate allometric scaling relationships between left ventricular mass (LVM) and height, total lean mass, and body surface area in healthy adolescents. The results suggest that pooled data without adjustment for sex may introduce bias in allometric scaling estimates. Clinicians should consider body size, body composition, sex, and age when assessing left ventricular mass.

Mesalazine-induced myopericarditis: a case series

Valentina Andrei, Nicoletta D'Ettore, Valentina Scheggi, Carlo di Mario

Summary: This article reports two cases of myopericarditis caused by hypersensitivity to 5-ASA, with cardiac magnetic resonance confirming the diagnosis. This rare but potentially life-threatening side effect is associated with mesalazine, a key therapeutic agent in IBD. Discussion highlights the difficulty of distinguishing between mesalazine-induced cardiac injury and acute IBD-induced cardiac inflammation.

Pulse wave velocity during re-feeding and with weight gain in underweight female adolescents with anorexia nervosa

Lee D. Hudson, Hind Al-Khairulla, Matthew Maicoo, Mario Cortina Borja, Alicja Rapala, Russell Viner, Dasha Nicholls, Andrew Taylor, Vivek Muthurangu, Alun Hughes

Summary: This study found that Anorexia Nervosa (AN) causes harmful underweight and important cardiovascular acute complications, but less is known about longer-term cardiovascular risk. The study measured carotid femoral pulse wave velocity (PWV) in underweight young women with AN at baseline and weekly during refeeding and weight gain. The results showed that PWV decreased over time and was negatively associated with increasing BMI and calorific meal content. These findings suggest potential positive cardiovascular benefits for refeeding and weight gain in AN, supporting the importance of weight gain in underweight young women with AN.
