Summary: This case presents a pregnant woman with systemic lupus erythematosus (SLE) who was diagnosed with asymptomatic complete heart block (CHB) during pregnancy. A multidisciplinary counselling board was held to evaluate the risks and benefits of pacemaker (PM) implantation, and it recommended the procedure to prevent complications and ensure sufficient cardiac output for intra-uterine growth. The patient underwent the procedure without complications and gave birth to a healthy baby.

Summary: Neonatal lupus erythematosus (NLE) is an autoimmune disease caused by the transplacental passage of anti-Ro/SS-A and anti-La/SS-B, with U1-ribonucleoprotein (U1RNP) also potentially involved. In a case involving a 7-day-old male, seizures and an irregular heart rhythm were detected, with the mother having systemic lupus erythematosus (SLE) but lacking follow-up by a rheumatologist. A workup for NLE revealed a negative anti-Ro/SS-A and anti-La/SS-B, but a positive U1RNP-70kD. The significance of U1RNP-70kD in NLE requires further research to improve care for mothers and babies.

Understanding the underlying mechanisms of NPSLE may lead to new treatment options, as shown in mouse models. However, current conventional therapies are only supported by observational studies, highlighting the need for controlled clinical trials.

Summary: Systemic lupus erythematosus (SLE) myocarditis is rare but has adverse outcomes. It is often difficult to recognize due to unspecific clinical presentation and a lack of data in the literature. Early detection of abnormalities in myocardial wall thickness and contractility was achieved using echocardiography, and prompt treatment for heart failure and immunosuppression resulted in a good response. Echocardiographic findings, biomarkers for myocardial injury (NT-proBNP, hs-troponin I), biomarkers for systemic inflammation (ESR, CRP), and biomarkers for SLE disease activity (Complement C3, C4, anti-dsDNA levels) guided the treatment.

Summary: This study aimed to identify subclinical right ventricular alterations in childhood-onset systemic lupus erythematosus (c-SLE) patients using conventional and three-dimensional echocardiography (3DE). The study found that c-SLE patients had larger end-systolic and end-diastolic volumes, as well as lower 3D-derived right ventricular ejection fraction, septal, and lateral longitudinal strain. These parameters correlated with SLE disease activity and chronicity scores.

Summary: This study evaluated global myocardial work (MW) in systemic lupus erythematosus (SLE) patients with normal left ventricular ejection fraction (LVEF) using two-dimensional speckle-tracking imaging (2D-STI). The results showed that SLE patients had higher global wasted work (GWW) and lower global work efficiency (GWE) compared to healthy controls, indicating the potential use of these parameters for the early detection of subclinical left ventricular dysfunction.

Summary: In recent years, there have been efforts to improve the prognosis and outcome in patients with autoimmune inflammatory rheumatic diseases by using treat-to-target strategy and early intervention strategies with immunosuppressive agents. However, infectious complications due to medication side effects remain a major concern. Vaccination programmes and vaccine immunity play a crucial role in patients with SLE in terms of morbidity and mortality. Although research on vaccines' efficacy and immune response in immunosuppressed patients has increased, there is still a lack of data investigating these parameters for those receiving biological agents, leading to a lack of consensus on a vaccination policy for patients with SLE.

Summary: Transplacental passage of maternal anti-SSA and anti-SSB antibodies, potentially associated with maternal autoimmune diseases, can cause neonatal lupus syndrome. This study examined the short- and long-term outcomes of mothers of offspring with congenital heart block (CHB). The results showed that one-quarter of the patients with CHB had an autoimmune disease diagnosis at the time of the fetal CHB diagnosis. Nearly half of those without an initial diagnosis progressed during follow-up, most without severe manifestations.

Summary: This study reviewed patient cohorts and populations to investigate the progression of cutaneous lupus to systemic lupus. The study found variations in the progression rates between adult and pediatric groups, which were attributed to differences in patient populations, study design, diagnostic criteria, and follow-up time. Risk factors associated with the development of systemic lupus included positive anti-nuclear antibodies, hematologic abnormalities, and a higher number of lupus classification criteria at baseline.

Summary: This study developed a selenium nanosystem with surface decoration of calf thymus DNA to specifically recognize and remove anti-dsDNA autoantibodies in SLE patients. The nanotherapeutics showed excellent biocompatibility and safety, and had higher clearance ratio compared to other clinical products. Furthermore, the nanotherapeutics could visualize the distribution of anti-dsDNA antibodies in kidney lesions and semi-quantify the antibody deposition.
