Improved Preservation and Microcirculation with POLYSOL After Partial Liver Transplantation in Rats

Background. Due to the severe shortage of deceased donors, demand for living-donor liver transplantation (LDLT) has increased worldwide. Here, we compared POLYSOL, a recently developed low-viscosity preservation solution, and histidine-tryptophan-ketoglutarate (HTK) for cold storage of partial liver graft in this study. Methods. Partial liver transplantations with 30% of the native liver were performed in Lewis rats. The graft livers were flushed with either HTK or POLYSOL (n = 25, respectively) and stored in the respective solution for 3 h at 5 degrees C. Graft function was evaluated regarding ischemia-reperfusion injury and regeneration at 1, 3, 24, and 168 h after reperfusion. Results. POLYSOL preservation resulted in improvement of portal venous flow (HTK versus POLYSOL; mean +/- SEM: 16.8 +/- 2.2 versus 21.6 +/- 2.1 mL/min; P = 0.005), microcirculation (383 +/- 63 versus 532 +/- 64 Flux; P = 0.045), ALT (310.2 +/- 56.1 versus 181.8 +/- 17.0 IU/L; P = 0.0262), LDH (4052.4 +/- 764.4 versus 2494.1 +/- 410.0 IU/L; P = 0.0215), total bilirubin (21.6 +/- 14.2 versus 4.0 +/- 0.6 IU/L; P = 0.0236), malondialdehyde (100.0 +/- 4.3 versus 69.2 +/- 4.0 nmol/mL; P = 0.0015), as well histologic findings at 24 h. Liver regeneration was improved in POLYSOL with regards to liver weight (4.0 +/- 0.2 versus 4.3 +/- 0.3 g; P = 0.038) and Ki-67 labeling index (9.67 +/- 2.17 versus 1.10 +/- 0.14%; P < 0.0001) at 24 h with higher up-regulation of portal VEGF (31.55 +/- 5.78 versus 91.94 +/- 9.27 pg/mL; P = 0.0052). Conclusions. This study showed that POLYSOL improves microcirculation and thus improves the preservation quality of partial liver transplantation. (C) 2011 Elsevier Inc. All rights reserved.

Assessment of reports on cadaver surgical training (CST) implementation in Japan: current status and challenges

Summary: This article reports on the contents and conflicts of interest of Cadaver Surgical Training (CST) programs in Japan based on professional autonomy. The Japan Surgical Society CST Promotion Committee evaluated the validity of the reports from participating universities and provided advice on proper implementation of CST. Over the past decade, 38 universities have been using cadavers for clinical education and research, such as CST. In case of identified inappropriate entries or unclear areas, the universities could be asked to resubmit their reports. This review clarifies the evaluation points of the CST reporting system, presents the status and problems of its reporting, and provides suggestions for future dissemination of CST.

Chronological changes in the gut microbiota and intestinal environment in recipients and donors of living donor liver transplantation

Summary: This prospective study investigated the changes in gut microbiota and associated intestinal environment during the early period after liver transplantation. The recipients showed dysbiosis and depletion of organic acid before transplantation, which were related to the severity of liver disease. The transplantation procedure caused minimal changes in gut microbiota and organic acid concentrations in the donors, while the recipients exhibited delayed recovery in these factors.

Evaluation of the ability of human induced nephron progenitor cells to form chimeric renal organoids using mouse embryonic renal progenitor cells

Summary: The number of patients with end-stage renal failure is increasing annually worldwide, but there is a shortage of renal transplantation donors. Transplanting renal progenitor cells into heterologous fetuses has been shown to induce the development of nephrons. In this study, researchers successfully generated human-mouse chimeric kidneys by combining this technology with drug-induced removal of specific renal progenitor cells in transgenic mice. They also developed a model using re-aggregation of mouse renal progenitor cells to evaluate the chimerogenic potential of human nephron progenitor cells derived from pluripotent stem cells. This study provides promising potential for the evaluation and generation of chimeric kidneys for regenerative medicine.

Orthotopic models showed bright and specific labeling with more than ten times higher tumor-to-background ratios compared to the control. This tumor-specific fluorescent antibody has promising potential as a clinical tool for improving visualization of gastric cancer margins during surgical resection.

Use of Broadband Personality Tests in Bariatric Surgery: A Scoping Review

Summary: This scoping review analyzed the literature on the use of broadband personality tests in the bariatric surgical population to optimize weight loss outcomes. The study found significant associations between personality scales and weight loss, but inconsistent reporting of outcome measures made it challenging to draw concrete conclusions. The American Society for Metabolic and Bariatric Surgery recommends standardization of outcome reporting to improve the reliability of predicting weight loss outcomes.
